肾移植患者急性排斥反应中T淋巴细胞亚群的作用研究

【摘要】 目的探讨外周血T淋巴细胞亚群变化在同种异体肾移植术后急性排斥反应中的意义。方法选取2009年至2010年在我院行肾移植的患者27例,术后按照是否发生急性排斥反应分为急性排斥反应组(n=11)和稳定组(n=16),分别于移植术前1 d及术后第7天收集患者外周血,采用流式细胞术分选CD3~+、CD4~+、CD8~+、CD3~+CD4~+及CD3~+CD8~+T细胞等,同时观察两组肾功指标及肾组织病理改变。结果与术前相比,急性排斥反应组及稳定组CD3~+CD4~+T淋巴细胞明显升高(P<0.05),而在急性排斥反应组和稳定组间无统计学差异(P>0.05)。结论监测患者外周血T淋巴细胞亚群的变化,对早期判别急性排斥反应及个体化免疫抑制剂治疗具有一定指导意义。

【Abstract】 Objective To study the value of peripheral blood T-lymphocyte subsets analysis in the diagnosis of acute rejection in human renal transplantion. Methods Selected 27 patients which did renal transplant in our hospital during 2009 to 2010, named preoperative group. 11 patients were diagnosed as acute graft rejection, and 14 patients were stable group. Among them, the levels of CD3~+, CD4~+, CD8~+, CD3~+CD4~+T and CD3~+CD8~+T cells in peripheral blood were measured and analyzed by flow cytometry at baseline(the 1st day before renal transplantation) and at the 7th day after renal transplantion, and date from conventional laboratory techniques were used for assessing renal function. Results The level of CD3~+CD4~+T in the acute graft rejection group and stable group were higher than preoperative group significantly(P<0.05), but between the acute graft rejection group and stable group there was no significance(P>0.05). Conclusions The monitoring of peripheral blood T-lymphocyte subsets was of significance in the diagnosis of acute rejection and the individualized immunosuppressive therapy for kidney transplant recipients.
