宁夏致泻性大肠埃希菌的流行特征及耐药现状研究

【摘要】 目的了解宁夏致泻性大肠埃希菌(dia rrheagenic E.coli)菌群流行特点及耐药趋势,为宁夏腹泻病的预防与控制提供重要依据。方法 2010—2014年收集宁夏腹泻患者粪便2910份,接种麦康凯培养基,挑取可疑菌落,采用PCR做鉴定与分型;Kirby-Bauer法进行药敏实验。结果 2910例腹泻患者共检出316株DEC,检出率为11.07%,以中年为主,青铜峡、中卫DEC检出率较高。银川、平罗和海原地区优势菌型为EIEC,中卫地区为EHEC和ETEC,青铜峡地区为EPEC、EHEC和ETEC。药敏试验中,除对氯霉素、庆大霉素、头孢曲松和头孢噻肟敏感外,对其他常用抗生素均耐药。结论宁夏地区由DEC感染严重,且不同地区、不同年份DEC的分布趋势、耐药性及致泻性特点均有不同,应引起足够的重视,建立长期持续性的DEC监测机制。

【Abstract】 Objective To investigate the epidemiological characteristics and antimicrobial susceptibility of diarrheagenic E.coli in Ningxia and to provide evidence for diarrhoeal prevention and control. Methods 2910 stool samples of diarrheal patients from 2010 to 2014 were collected and cultured on MAC agar; the suspicious strains were isolated, the isolates were identified by PCR; then the Kirby-Bauer assay was used to test the antimicrobial susceptibility. Results 316 diarrheagenic E.coli strains were found in 1520 patients, the prevalance of DEC was 11.07%, the middle-aged patients, Qingtongxia and Zhongwei areas with DEC were higher than other groups. the advantage bacterium in different areas of Ningxia were diverse; it was EIEC in Yinchuan, Pingluo and Haiyuan; EHEC and ETEC in Zhongwei; Qingtongxia was EPEC, EHEC and ETEC. In addition to chloram phenicol, gentamicin, cefatriaxone and cefotaxime, the isolates were resistance to other common antibiotics in this experement.Conclusion The diarrhea caused by DEC in Ningxia was serious, the distribution trend, drug resistance and diarrhea characteristics in different time and areas was various, so the long-term sustainability monitoring mechanism of DEC should be set up.
