【摘要】 2009年,德国的Hohenberger教授首次提出了结肠癌完整结肠系膜切除术(complete mesocolic excision,CME)理念。完整结肠系膜切除手术依照胚胎发育解剖层面锐性分离并完整切除结肠系膜和肿瘤淋巴引流区域,同时高位结扎供应血管,从而达到清扫淋巴结的最大化,以期降低结肠癌局部复发率和提高生存率。CME手术已经逐渐成为结肠癌规范化手术方式。本视频是一乙状结肠癌患者手术过程,术者全程沿着结肠脏层筋膜和壁层筋膜的间隙锐性分离,手术解剖层次清晰,术中出血极少。切除标本的质量分级可评定为系膜层面。本视频有助于结直肠外科医师对CME手术的理解,并为进行同类手术提供参考。更多还原

【Abstract】 In 2009,Prof. Hohenberger proposed the concept of complete mesocolic excision( CME) for colon neoplasms. CME attempts to remove the entire mesocolon and all potential routes of lymph spread by carefully dissecting along embryologic tissue planes and transecting the supplying vessels at their origin,so as to harvest optimal lymph nodes,eventually decrease local recurrence,and increase the survival rate. CME has become the standardized surgery for colon cancer. This video demonstrats an operation for a patient diagnosed with sigmoid colon cancer. The surgeon separates the visceral plane from the parietal one by careful and sharp dissection during the operation,with clear anatomic structure and little bleeding. The resected specimen was assessed of high quality,and defined as a mesocolon plane. This video may enhance the understanding of CME technique and offer a paradigm for colorectal surgeons.更多还原
