不同面积胸壁浮动对犬心肺功能的影响
The influence of different sized floating thoracic wall on dogs cardiopulmonary function
【摘要】 目的:观察不同面积胸壁浮动对犬心肺功能的影响以及胸壁加压包扎的疗效.方法:实验用杂交犬18只建立大、中和小面积三组胸壁浮动动物模型,用胸腔置管、SwanGanz导管和血气分析等观察胸膜腔内压(IPP)、平均动脉压(MAP)、中心静脉压(CVP)、动脉血氧分压(PaO2)、动脉血二氧化碳分压(PaCO2)及动脉血氧饱和度(SaO2)等变化和加压包扎治疗的效果.结果:大、中面积组MAP、PaO2和SaO2下降(P<0.05),CVP和PaCO2升高(P<0.05);小面积组CVP升高(P<0.05),MAP和SaO2下降(P<0.05).同模型制作后比较,加压包扎治疗后,大、中面积组心肺功能指标差异无显著性意义;小面积组加压包扎治疗后,PaO2和SaO2上升(P<0.05),CVP和PaCO2下降(P<0.05).结论:大、中面积连枷胸对心肺功能影响较大,加压包扎治疗无效.小面积连枷胸对心肺功能也有一定影响,加压包扎治疗对心肺功能有一定的改善.
【Abstract】 AIM:To study the influence of different sized floating thoracic wall on dogs cardiopulmonary function and the curative effect of pressure dressing on chest.METHODS:Floating thoracic wall models of large,medium and small size were established in 18 hybrid dogs.IPP,MAP,CVP,PaO2,PaCO2 and SaO2 were measured with Swan-Ganz catheters,blood gas analysis and intrathoracic cannula.And the curative effect of pressure dressing on chest on cardiopulmonary function was studied too.RESULTS:MAP,PaO2 and SaO2 decreased(P<0.05),but CVP and PaCO2 increased(P<0.05)in large and medium size group.MAP and SaO2 decreased(P<0.05),but CVP increased(P<0.05)in small size group.PaO2 and SaO2 increased(P<0.05),CVP and PaCO2 decreased(P<0.05)in small size group after treatment of pressure dressing on chest.CONCLUSION:The flail chest of large and medium size group had a great effect on cardiopulmonary function.The pressure dressing on chest was effective way for improvement of cardiopulmonary function in flail chest of small size.
