上海市闵行区2013年HIV-1分子流行病学研究

【摘要】 目的了解上海市闵行区艾滋病病毒(HIV)感染者特征和HIV基因亚型情况。方法上海市闵行区2013年首次确诊为HIV阳性的感染者血清,应用巢氏PCR扩增pol基因,使用HIV-Blast软件判定亚型;Mega 5.1软件将待分析序列与国际参考株序列比对,采用Neighbor-Joining(N-J)法构建系统进化树。应用SPSS 17.0软件,对计数数据采用χ2检验和Fisher′s精确概率计算法进行比较,P<0.05为差异有统计学意义。结果有101例感染者纳入研究,成功扩增86例(85.15%)患者的pol基因。共发现6种亚型:CRF01_AE、CRF07_BC、RF08_BC、B、CC和CRF01_AE/B。CRF01_AE是最主要的亚型(60.47%,52/86),其次是CRF07_BC(27.91%,24/86)。感染者均由性途径传播,其中同性传播占63.95%(55/86),异性传播占36.05%(31/86)。结论同性传播是闵行区HIV感染的主要传播途径。流动人口聚集,带来的流行毒株多样化已经成为闵行区HIV的流行特点,应加强对HIV亚型变异的监测。

【Abstract】 Objective To analyze HIV subtype distribution among newly-diagnosed HIV infected individuals in Minhang District,Shanghai. Methods In 2013,all newly-diagnosed HIV-infected adults in Minhang District,Shanghai were tested for HIV subtype by amplifying and sequencing HIV-1pol gene.HIV-1subtype reference sequences available at the Los Alamos HIV sequences database were used for HIV-1subtyping.Molecular Evolutionary Genetics Analysis(MEGA)software was used for Phylogenetic analysis.Statistical analysis was performed with SPSS version 17.0(SPSS Inc.,USA).Categorical variables were compared usingχ2 test or Fisher′s exact test.All tests were two-tailed and a P value<0.05 was considered as significant. Results Out of the 101newly-diagnosed HIV-infected adults,86(85.15%)were successfully genotyped.A total of 6subtypes were identified,including CRF01_AE,CRF07_BC,CRF08_BC,B,C and CRF01_AE/B.The predominant HIV-1subtype was CRF01_AE(60.47%,52/86),followed by CRF07_BC(27.91%,24/86).All newly-diagnosed HIV adults contracted virus via sexual contact,and heterosexual transmission contributed for 63.95%(55/86)of total infection. Conclusions Heterosexual transmission is the major way of HIV infection in Minhang district.HIV subtype diversity may be the consequence of huge migrating population in the area and HIV subtype surveillance should be strengthened.
