【摘要】 目的探讨精细化被膜解剖法在甲状腺手术中的应用效果。方法选择2012年1月~2013年12月在我院接受甲状腺手术的258例患者作为研究对象,随机分为对照组和观察组,各129例。对照组患者采用传统甲状腺术式治疗,观察组患者采用精细化被膜解剖法,结合裸化血管、整体切除等一系列技术理念进行甲状腺手术,比较两组手术效果。结果观察组低钙血症发生率(0.78%)显著低于对照组(6.20%),组间比较差异有统计学意义(P<0.05)。观察组暂时性喉返神经损伤、永久性喉返神经损伤、术后出血、永久性甲状旁腺功能低下发生率显著低于对照组,两组比较差异有统计学意义(P<0.05)。观察组切口感染发生率与对照组比较,差异无统计学意义(P>0.05)。结论以精细化被膜解剖法为核心的甲状腺手术技术安全、可靠,值得临床推广。更多还原

【Abstract】 Objective To explore the application effect of meticulous capsular dissection technique in thyroid surgery.Methods 258 patients who undergone thyroid surgeries from January 2012 to December 2013 in our hospital were selected as research objects and evenly divided into control group and observation group in random.In the control group,traditional thyroid surgery was used,while in the observation group,a series of technical concepts including meticulous capsular dissection technique together with bare blood vessel and overall removal for thyroid surgery were adopted.The surgical effect between two groups was compared. Results The incidence of hypocalcemia in observation group was0.78%,greatly lower than that in control group accounting for 6.20%,with statistical difference(P <0.05).Incidence of complications such as temporary laryngeal recurrent nerve injury,permanent laryngeal recurrent nerve injury,postoperative hemorrhage,and permanent parathyroid hypofunction in observation group was significantly lower than that in control group,with statistical difference(P<0.05).With regard to incidence of incision infection,there was no significant difference between observation group and control group(P>0.05). Conclusion Thyroid surgery centered on meticulous capsular dissection technique is safe and reliable,which is worthy of clinical promotion.更多还原
