【摘要】 目的研究卡前列素氨丁三醇防治产妇产后出血的临床效果。方法 2011年1月~2014年1月,选择90例存在宫缩乏力性产后出血因素的产妇进行研究,按随机数字表法分为试验组(45例)与对照组(45例),两组产妇在胎儿娩出后均静脉滴注20 U缩宫素,在此基础上,试验组产妇宫肌注射或臀肌注射250μg卡前列素氨丁三醇,对照组产妇宫肌注射或臀肌注射20 U缩宫素。经处理后,比较两组产妇的产后2、24 h出血量,产后出血及不良反应发生情况。结果试验组产后2、24 h出血量及产后24 h血红蛋白下降值均显著少于对照组(P<0.05);试验组的产后出血发生率为11.1%,明显低于对照组(31.1%),两组比较,差异有统计学意义(χ2=5.4040,P=0.0201);试验组的不良反应发生率为13.3%,对照组为8.9%,两组比较,差异无统计学意义(χ2=0.4500,P=0.5023)。结论卡前列素氨丁三醇防治产妇产后出血的效果良好,安全性高,值得临床推广应用。更多还原

【Abstract】 Objective To study the clinical effect preventing and treating of postpartum hemorrhage of puerpera.Methods From January 2011 to January 2014,90 puerperae with postpartum hemorrhage due to uterine inertia were selected and evenly divided into experimental group and control group by random number table method.After delivery,20 U oxytocin was applied by intravenous drip in both groups.On the basis,250 μg carboprost tromethamine was injected by uterus muscle or gluteus in experimental group.In control group,20 U oxytocin was injected by uterus muscle or gluteus.After treatment,the amount of bleeding after 2 hours and 24 hours delivery,occurrence of postpartum hemorrhage and adverse reaction between two groups were compared. Results Amount of bleeding after 2 hours and 24 hours,and the decreased value of hemoglobin after 24 hours delivery was greatly lower than that in control group respectively(P <0.05).The incidence rate of postpartum hemorrhage in experimental group(11.1%) was lower in comparison with that in control group(31.1%) with a statistical difference(χ2=5.4040,P=0.0201).The incidence rate of adverse reaction in experimental group(13.3%),while it was 8.9% in control group,and there was no statistical difference between two groups(χ2=0.4500,P=0.5023). Conclusion Carboprost tromethamine can obtain a good effect on preventing and treating of postpartum hemorrhage of puerpera with high safety,which is worthy of clinical promotion and application.更多还原
