艾司洛尔对甲状腺功能亢进症患者全身麻醉心血管副反应的影响
Influence of esmolol on cardiovascular side effect in patients with hyper-thyroidism after general anesthesia
【摘要】 目的观察艾司洛尔对甲状腺功能亢进症患者全身麻醉心血管副反应的影响。方法随机选取本院2013年12月~2014年12月行甲状腺大部分切除术的患者80例,将其分为艾司洛尔组(实验组)和生理盐水组(对照组),各40例。对比两组的血压、心率等变化及手术时间、术中出血量、术后并发症发生情况等。结果气管插管、切皮、术程、拔管时,实验组的血压、心率均较为平稳,对照组的血压、心率则出现较大波动,两组比较,差异有统计学意义(P<0.05);实验组的手术时间为(91.3±17.1)min,明显短于对照组的(126±23.5)min,术中出血量为(24.7±4.6)ml,明显少于对照组的(32.4±8.3)ml,两组比较,差异有统计学意义(P<0.05)。实验组仅1例出现并发症,对照组为17例,两组比较,差异有统计学意义(P<0.05)。结论对行甲状腺大部分切除术的患者围术期应用艾司洛尔可大大提高麻醉管理质量,使患者术中的血压、心率稳定,减少了术野渗血,并缩短了手术时间。
【Abstract】 Objective To observe the influence of esmolol on cardiovascular side effect in patients with hyperthyroidism after general anesthesia. Methods 80 patients with resection of the majority of thyroid gland in our hospital from December 2013 to December 2014 were randomly selected,and they were evenly divided into esmolol group(experimental group) and saline group(control group).The change of blood pressure,heart rate,and operation time,intraoperative amount of bleeding,and incidence of postoperative complication in two groups were compared. Results During tracheal intubation,skin incision,course of treatment,and extubation,the blood pressure and heart rate in experimental group were relatively stable,while in control group,these two indexes displayed great change with a statistical difference after comparison(P <0.05).The operation time in experimental group [(91.3 ±17.1) min] was obviously shorter than that in control group [(126±23.5) min].The intraoperative amount of bleeding in experimental group [(24.7 ± 4.6) ml] was obviously fewer than that in control group [(32.4±8.3)ml] with statistical differences(P<0.05).There was only one case occurred with complication in experimental group,while in control group, the number of cases occurred with complications was17,which displayed a statistical difference(P<0.05). Conclusion For patients with resection of the majority of thyroid gland during perioperative period,esmolol applied can greatly improve the quality of anesthesia management,stabilize patients′ blood pressure and heart rate,and alleviate bleeding in surgical site as well as shorten the operation time.
