【摘要】 目的观察彩色多普勒超声在乳腺小肿块良恶性鉴别中的应用价值。方法选取2010年5月~2014年11月收治的乳腺小肿块患者115例,经手术病理鉴别诊断,其中55例为乳腺癌(恶性组)、60例为乳腺良性肿块(良性组),对比两组的术前彩色超声成像中血流信号情况及超声诊断结果。结果恶性组的收缩期血液流速峰值明显快于良性组、阻力指数明显高于良性组,恶性组少血流信号(0~Ⅰ级)的比例明显低于良性组(P<0.05);恶性组以穿入型血流为主,良性组以环绕型血流为主,两组的血流形态学分型结果差异有统计学意义(P<0.05)。结论良恶性乳腺小肿块在彩色超声显示中具有不同的血流信号特点,血流的相关参数与定量分布情况对乳腺小肿块的良恶性鉴别具有一定的提示作用,可以辅助诊断良恶性乳腺小肿块。更多还原

【Abstract】 Objective To observe the application value of color Doppler ultrasound in the identification of benign and malignant small tumor of breast. Methods 115 patients with small tumor of breast treated from May 2015 to November2014 were selected,and they were diagnosed by the differential diagnosis of operation and pathology,and among them,55 cases were breast cancer(the malignant group),60 cases were benign tumor of breast(the benign group),the preoperative blood flow signal of color ultrasound imaging and ultrasound diagnosis results in the two groups were compared.Results The peak value of systolic blood flow velocity in the malignant group was significantly faster than that in the benign group,the resistance index in the malignant group was significantly higher than in the benign group,and the ratio of less blood flow signal(0 to grade Ⅰ) in the malignant group was significantly lower than that in the benign group(P <0.05);malignant group to the entering flow based, benign group to surround type flow based, statistically significant flow morphology two groups of genotyping results of difference(P < 0.05).Entering blood flow was main in the malignant group,type surround blood flow was main in the benign group,and there was a statistical difference of the type of morphology classification of blood flow(P < 0.05). Conclusion Benign and malignant small tumor of breast has different blood flow signal characteristic in color ultrasound display,the related parameter and quantitative distribution of blood flow has a certain suggestive role for the identification of benign and malignant small tumor of breast,and it can help the diagnosis of benign and malignant small tumor of breast.更多还原
