【摘要】 目的观察He-Ne激光穴位深部与体表照射及单纯针刺穴位治疗继发性坐骨神经痛的疗效。方法继发性坐骨神经痛患者166例,按就诊顺序随机分为三组。(1)深部照射组:患者56例,采用特制空芯针灸针针刺患侧L4、5夹脊和环跳穴,将直径50μm光纤通过针芯导入穴位后,行He-Ne激光穴位深部照射,功率密度382 m W/cm2,时间10min,每日1次,5 d为1个疗程,间隔3 d行下个疗程,均治疗3个疗程,无效者改行其他方法;(2)体表照射组:患者56例,采用He-Ne激光直接行穴位体表照射,光纤直径600μm,激光功率密度、照射时间及疗程均同深部照射组;(3)单纯针刺组:患者54例,采用单纯针刺穴位,除无激光照射外,其他均同深部照射组。观察三组患者的症状、体征改善情况和年龄、病程、病因对疗效的影响,治疗后3个月复查评定疗效。结果深部照射组56例,治愈35例(占62.5%),好转17例(占30.4%),无效4例(占7.1%);体表照射组56例,治愈27例(占48.2%),好转11例(占19.6%),无效18例(占32.2%),单纯针刺组54例,治愈25例(占46.3%),好转12例(占22.2%),无效17例(占31.5%)。深部照射组疗效好于体表照射组和单纯针刺组(P<0.05)。结论 He-Ne激光穴位深部照射治疗继发性坐骨神经痛具有较好的疗效,此疗法既有传统的针刺作用又有激光照射的生物刺激效应,为临床治疗继发性坐骨神经痛提供了一条新途径。更多还原

【Abstract】 Objective To compare the effect of inside irradiation and surface irradiation of He-Ne laser and simple acupuncture of acupoints on secondary sciatica.Methods Totally 166 patients with secondary sciatica were randomly divided into three groups: the inside irradiation group with 56 patients,the surface irradiation group with 56 patients and the simple acupuncture group with 54 patients. For the inside irradiation group,special hollow acupuncture needles were introduced into such acupoints as Jiaji( EX-B2) L4 and 5 and Huantiao( GB30) on the affected side. Then light guide fiber of He-Ne laser needles were inserted into the acupoints through the hollow acupuncture needles for inside irradiation( power: 3 m W,diameter of light spot: 0. 1 cm and density of power: 382 m W / cm2) of 10 minutes per day. Each course of treatment lasted for 5 days and 3 courses were given at an interval of 3 days. Except of the irradiation location being the surface of the acupoints,the patients in the surface irradiation group were given the same He-Ne laser irradiation as that for the inside irradiation group,including the choice of acupionts,power density of laser,times of irradiation and courses of treatment. Except of the laser irradiation,the patients in the simple acupuncture group were given acupuncture on the same acupoints on the same conditions as those in the inside irradiation group. The patients in all three groups were given three courses of treatment. The therapy was changed once it turned out ineffective. The changes to symptoms and signs of the patients and the influence of age,and course and cause of disease on curative effect were observed in the three groups. The patients of all the three groups were reexamined to evaluate the therapeutic effect three month after the treatment.Results In the inside irradiation group,35 patients( 62. 5%) were cured,17 patients( 30. 4%) improved and 4 patients( 7. 1%)unimproved. For the surface irradiation group,27 patients( 48. 2%) were cured,11 patients( 19. 6%) improved and 18 patients( 32. 1%) unimproved. As to the simple acupuncture group,25 patients( 46. 3%) were cured,12 patients( 22. 2%) improved and17 patients( 31. 5%) unimproved. The effect of inside irradiation group was remarkably better than that of the surface irradiation group and simple acupuncture group( P < 0. 05).Conclusions Inside irradiation of acupoints with He-Ne laser is better than surface irradiation as well as simple acupuncture in treating secondary sciatica. Combining the effect of traditional acupuncture and the stimulative effect of biology of laser irradiation,it provides a new solution to secondary sciatica.更多还原
