【摘要】 目的探讨不同病理分期肾透明细胞癌(CCRCC)的超声造影(CEUS)特点。方法回顾性分析经病理证实的85例CCRCC的CEUS图像及相关参数,观察T分期中不同肿瘤的CEUS增强模式、程度、增强的均匀性及肿瘤周边环状高增强(PHR);通过时间-强度曲线软件得到数值,包括始增时间(AT)、达峰时间(TTP)和峰值强度(PI),并与病理分期进行比较。为避免深度、个体循环容积等差异对信号强度的影响,使PI标准化,采用△PI,即肾细胞癌增强强度与正常肾实质增强强度的差值,可定量显示肿瘤的微循环灌注。结果 pT1期肿瘤中PHR检出率高,△PI最低;pT3期肿瘤PHR检出率低,△PI最高。各期肿瘤增强均匀性差异均有统计学意义(P均<0.05)。肿瘤CEUS增强模式大多表现为"快进"(64/85,72.94%)、"快出"(47/85,42.35%)和"慢出"(35/85,30.59%),不同分期肿瘤造影模式差异无统计学意义(P>0.05)。结论 CCRCC的CEUS增强特点与肿瘤分期有相关性,CEUS可为临床提供有价值的诊断信息。更多还原

【Abstract】 Objective To analyze and comparice of different pathological stages of CCRCC features of contrast enhanced ultrasound.Methods Retrospective analysis of the CEUS images and related parameters of 85 patients with CCRCC confirmed by pathology,enhance the uniformity and tumor peripheral annular enhancement observation of different T CEUS staging of tumor,the degree and pattern of enhancement(PHR);get the quantitative value by the time intensity curve software,including AT,TTP and PI,and T and pathological staging were compared.ResultsThe detection rate of PHR is relatively high T stage pT1 tumors,Delta PI minimum;grade pT3 tumor PHR detection rate is relatively low,the highest Delta PI.The tumor improve the uniformity of the differences were statistically significant(P<0.05).Tumor CEUS enhancement mode mostly for "fast forward"(64/85,72.94%), "fast"(47/85,42.35%)and "slow out"(35/85,30.59%),no significant difference in different stages of tumor imaging mode(P>0.05).Conclusions The characteristics of CEUS and CCRCC stage are correlation.更多还原
