【摘要】 目的探讨超声心动图对部分性肺静脉异位引流(PAPVC)的诊断价值。方法回顾性分析29例PAPVC患者二维超声及彩色多普勒图像表现并与心血管造影、手术结果对比分析,总结其诊断特点,找出漏诊的原因。结果术前确诊25例,漏诊3例,1例提示诊断。右上肺静脉引流入右心房19例;右上肺静脉引流入上腔静脉2例;右上肺静脉及右下肺静脉均引流入右心房2例;右下肺静脉引流入右心房2例;右上肺静脉及右下肺静脉引流入冠状静脉窦1例;左上及左下肺静脉通过垂直静脉引流入左无名静脉3例。合并房间隔缺损(ASD)27例,其中静脉窦型ASD 20例,继发孔ASD 7例,1例房间隔完整,1例合并卵圆孔未闭。25例超声诊断与心血管造影、手术结果符合。结论超声心动图诊断PAPVC具有较高的准确度,但在成人时期容易漏诊。更多还原

【Abstract】 Objective To study the value of echocardiography for diagnosis of partial anomalous pulmonary venous connection(PAPVC).Methods The Echocardiographic characteristics of PAPVC in 29 cases were compared with results of cardiac catheterization and operation.The diagnostic features of echocardiography and misdiagnosis were summarized.Results 25 cases were confirmed and 3cases were misdiagnosed,1case was not confirmed.19 cases had a right upper pulmonary vein anomalously connected to the right atrium,2had a right upper pulmonary vein connected to the superior vena cava(SVC).Both right-sided veins connected to the right atrium in 2cases.2cases had a right lower vein connected to the right atrium.1case had two right-sided veins anomalously connected to the coronary sinus.In addition,there were 3anomalous both left pulomary veins that connected to the innominate vein by means of a vertical vein before communicating with the SVC.The most common associated anomaly was an atrial septal defect(ASD)in 27 cases,20cases having sinus venous ASD,7cases having secondary ASD,1case having patent foramen ovale(PFO).1case had an intact atrial septum.The echocardiographic characteristics of PAPVC in 25 cases were correspondent to the results of cardiac catheterization or operation.Conclusions There is high accuracy by using echocardiography to diagnosis PAPVC.If the PAPVC in adult period,the PAPVC were easy misdiagnosed.更多还原
