左乙拉西坦、奥卡西平对部分性发作癫痫患儿的临床疗效及脑电图的影响
Clinical effects of levetiracetam and oxcarbazepine for children with partial onset seizures and the impacts on electroencephalogram
【摘要】 目的探讨左乙拉西坦(LEV)、奥卡西平(OXC)单药治疗部分性发作癫痫患儿的疗效及对脑电图的影响。方法将确诊的癫痫部分性发作患儿108例根据治疗药物不同随机分成LEV组和OXC组各54例,分别给予LEV、OXC单药治疗12个月,观察疗效及治疗前后脑电图变化。结果 LEV组、OXC组治疗12个月后总有效率分别为68.3%、81.3%,差异无统计学意义(P>0.05)。LEV组治疗后70.7%病例发作间期癫痫样放电(IEA)消失或减少50%以上,而OXC组治疗后64.6%IEA消失或减少50%以上,两组比较差异无统计学意义(P>0.05)。两组治疗前及治疗12个月后脑电图背景活动均以α波活动为主,其次为θ波为主。且治疗前后脑电图背景活动自身比较及两组间比较差异均无统计学意义,(P>0.05)。结论 LEV、OXC单药治疗儿童癫痫部分性发作均有较好的效果,两种药物对IEA均有明显的抑制作用,且对脑电背景活动无影响。
【Abstract】 Objective To explore the clinical effects of levetiracetam( LEV) and oxcarbazepine( OXC) for children with partial onset seizures and the impacts on electroencephalogram. Methods A total of 108 children diagnosed as partial onset seizures definitely were randomly divided into LEV group and OXC group according to different therapeutic drugs,54 children in each group. The children in the two groups were treated by LEV and OXC respectively for 12 months; the curative effects and changes of electroencephalogram before and after treatment were observed. Results The total effective rates after 12 months of treatment in LEV group and OXC group were 68. 3% and81. 3%,respectively,there was no statistically significant difference between the two groups( P>0. 05). In LEV group,interictal epileptiform discharge( IEA) disappeared or decreased by more than 50% among 70. 7% of the children,while in OXC group,the proportion was64. 6%,there was no statistically significant difference between the two groups( P>0. 05). In the two groups,α wave activity was the main type of electroencephalogram background activities before and after 12 months of treatment,followed by θ wave activity. There was no statistically significant difference in electroencephalogram background activities between before and after treatment in the same group and between the two groups( P>0. 05). Conclusion Monotherapy of LEV and OXC can achieve good efficacy in treatment of children with partial onset seizures,LEV and OXC can significantly inhibit IEA and have no impact on electroencephalogram background activities.
