微刺激联合黄体期促排卵方案在卵巢低反应患者IVF-ET助孕中的应用

【摘要】 目的比较微刺激方案和微刺激联合黄体期促排卵方案在卵巢低反应患者体外受精-胚胎移植(IVF-ET)中的效果,探讨黄体期促排卵在卵巢低反应患者中的应用。方法回顾性分析2011年1月~2013年12月于河南省人民医院生殖医学研究所行微刺激方案的368个IVF-ET周期。根据促排卵方案的不同分为A组(微刺激方案,279周期)和B组(微刺激联合黄体期促排卵方案,89周期)。比较两组的基本情况、助孕结局和妊娠结局。结果 A组共144周期无可利用胚胎,B组共16周期无可利用胚胎。A组平均Gn量、平均促性腺激素(Gn)天数、获卵数、可移植胚胎数均显著低于B组(P<0.05);A组周期取消率显著高于B组(P<0.05)。A组和B组扳机日子宫内膜厚度、移植胚胎数、临床妊娠率、胚胎种植率及早期流产率比较差异无统计学意义(P>0.05);B组促排卵周期妊娠率高于A组(P<0.05)。结论微刺激联合黄体期促排卵方案可以提高卵巢低反应患者的获卵数及可移植胚胎数、降低周期取消率,可以达到理想的临床妊娠率,是卵巢低反应患者有效的促排卵方案。

【Abstract】 Objective To compare the effects of micro- stimulation and micro- stimulation combined with luteal- phase ovulation induction in in vitro fertilization- embryo transfer( IVF- ET) of patients with poor ovarian response( POR),explore the application of luteal- phase ovulation induction in patients with POR. Methods A total of 368 micro- stimulation IVF- ET cycles were analyzed retrospectively in Research Institute of Reproductive Medicine in He’nan Provincial People’s Hospital from January 2011 to December 2013,then they were divided into group A( micro- stimulation,279 cycles) and group B( micro- stimulation combined with luteal- phase ovulation induction,89 cycles). The basic situations,outcomes of IVF- ET,and pregnancy outcomes in the two groups were compared. Results In group A,144 cycles had no available embryos; in group B,16 cycles had no available embryos. The mean gonadotropin( Gn) dose,the mean application time of Gn,the number of retrieved oocytes,and the number of available embryos in group A were statistically significantly lower than those in group B( P < 0. 05); the cycle cancellation rate in group A was statistically significantly higher than that in group B( P< 0. 05); there was no statistically significant difference in endometrial thickness,the number of transferred embryos,clinical pregnancy rate,embryo implantation rate,and early abortion rate between group A and group B; the cycle pregnancy rate of ovulation induction in group B was statistically significantly higher than that in group A( P < 0. 05). Conclusion Micro- stimulation combined with luteal-phase ovulation induction can increase the numbers of retrieved oocytes and available embryos,reduce cycle cancellation rate,obtain ideal clinical pregnancy rate,which is an effective method for ovulation induction in POR patients.
