【摘要】 目的:评估腹腔镜下输卵管通液术对输卵管性不孕症妇女的临床诊断价值。方法:2013年1月~2014年12月该院妇科收治的120例拟诊断为输卵管性不孕症妇女作为研究对象,分别采用腹腔镜下输卵管通液检查及子宫输卵管碘比醇造影检查,并以腹腔镜探查术中所见作为"金标准",对两种方法诊断输卵管不孕症的临床价值进行比较。结果:120例不孕症患者术中所见:双侧输卵管畅通39例(32.50%),单侧/双侧不通81例(67.50%);其中盆腔炎性疾病72例(60.00%)、子宫内膜异位症患者24例(20.00%)、其他疾病16例(13.33%)、正常盆腔8例(6.67%)。子宫输卵管碘比醇造影检查诊断输卵管阻塞的灵敏度=84.62%、特异度=69.14%、Kappa=0.447、AUC=0.715,碘比醇造影检查与手术探查结果具有中等一致性。输卵管通液检查诊断输卵管阻塞的灵敏度=94.87%、特异度=83.95%、Kappa=0.735、AUC=0.904,输卵管通液检查与手术探查结果具有高度一致性。结论:腹腔镜下输卵管通液术对输卵管性不孕症妇女具有较高的诊断价值,诊断结果与手术结果具有高度的一致性,在临床上可以作为一种可靠的诊断手段。更多还原

【Abstract】 Objective: To evaluate the clinical diagnostic value of laparoscopic hydrotubation for tubal infertility. Methods: A total of 120 women suspected of tubal infertility in Department of Gynecology in the hospital from January 2013 to December 2014 were selected as study objects,then they underwent laparoscopic hydrotubation and hysterosalpingography,respectively,laparoscopic result was designed as gold standard; the clinical values of the two methods in diagnosis of tubal infertility were compared. Results: Laparoscopic findings: 39women( 32. 50%) were found with unblocked bilateral Fallopian tubes,81 women( 67. 50%) were found with unilateral or bilateral oviduct obstruction,72 women( 60. 00%) were found with pelvic inflammatory diseases,24 women( 20. 00%) were found with endometriosis,16women( 13. 33%) were found with other diseases,8 women( 6. 67%) were found with normal pelvic cavity. The sensitivity,specificity,Kappa value,and AUC of hysterosalpingography in diagnosis of oviduct obstruction were 84. 62%,69. 14%,0. 447,and 0. 715,respectively; the consistency of laparoscopy and hysterosalpingography was moderate. The sensitivity,specificity,Kappa value,and AUC of laparoscopic hydrotubation were 94. 87%,83. 95%,0. 735,and 0. 904,respectively; the consistency of laparoscopy and laparoscopic hydrotubation was high. Conclusion: The diagnostic value of laparoscopic hydrotubation for tubal infertility is high,the diagnostic results of laparoscopic hydrotubation and laparoscopy have high consistency,laparoscopic hydrotubation can be used as a reliable diagnostic tool for tubal infertility.更多还原
