地氯雷他定对皮肤划痕症患者组胺及Th1/Th2细胞平衡的影响
The Effect of Desloratadine on Histamine and Th1 / Th2 Balance in Patient with Dermatographism
【摘要】 目的观察地氯雷他定对皮肤划痕症患者组胺以及Th1/Th2细胞的影响,并探讨其相关的临床意义。方法收集北华大学附属医院皮肤性病科2012年6月-2014年1月治疗的皮肤划痕症患者40例,另取北华大学附属医院体检中心的健康体检者40例作为正常对照。Th1和Th2细胞使用流式细胞仪检测,组胺、Ig E,IL-4,IL-2,IFN-γ因子表达使用ELISA法检测。结果治疗前皮肤划痕症患者组胺含量、Ig E和IL-4表达水平显著高于对照组(P<0.01),Th1/Th2细胞比值、IL-2和IFN-γ表达水平显著低于对照组(P<0.01),治疗后组胺含量、Ig E和IL-4表达水平出现显著减低(P<0.01),Th1/Th2细胞比值、IL-2和IFN-γ表达水平显著升高(P<0.01)。Ig E表达与IL-4呈显著正相关(r=0.522,P<0.05),与IL-2呈显著负相关(r=-0.361,P<0.05),与IFN-γ呈显著负相关(r=-0.403,P<0.05)。地氯雷他定治疗皮肤划痕症临床有效率为85.0%,不良反应发生率为7.5%。结论皮肤划痕症患者中存在组胺高表达及Th1/Th2细胞的失衡,地氯雷他定可以抑制组胺表达,调节Th1/Th2细胞的失衡,安全、高效的治疗皮肤划痕症。
【Abstract】 Objective To observe the effect of Desloratadine on histamine and Th1 /Th2 cells in patient with dermatographism and explore its clinical efficacy. Methods A total of 40 cases of dermatographism patients were selected from the Affiliated Hospital of Beihua University. Another 40 cases of healthy persons were used as control. The percentage of Th1 and Th2 cells were detected by flow cytometry analysis. The expression of histamine,Ig E,IL-4,IL-2,and IFN-γ were determined by ELISA analysis. Results The expression of histamine,Ig E and IL-4 were higher in dermatographism patients than that in control( P < 0. 01). The ratio of Th1 / Th2,expression of IL-2 and IFN-γ were lower in dermatographism patients than that in control( P <0. 01). After Desloratadine treatment,the expression of histamine,Ig E and IL-4 were reduced( P <0. 01),and the ratio of Th1 / Th2,expression of IL-2 and IFN-γ were increased( P < 0. 01). Ig E expression was positively correlated with IL-4( r = 0. 522,P < 0. 05),negatively correlated with IL-2( r =- 0. 361,P < 0. 05),and negatively correlated with IFN-γ( r =- 0. 403,P < 0. 05). The rate of clinical effectiveness was 85% and the rate of adverse reaction was 7. 5%. Conclusion Overexpression of histamine and imbalance of Th1 / Th2 were found in dermatographism patients. Desloratadine can inhibit the histamine expression and adjust the imbalance of Th1 / Th2,which is safe and efficient for the treatment of dermatographism.
