【摘要】 患者女,16岁。面、上胸部、腰部、上臂、股及双手足背出现红斑、丘疹、丘疱疹、坏死、结痂和痘疮样瘢痕伴痒痛7年余,日晒后加重。皮损组织病理示:表皮增生,局部变性坏死,真皮乳头水肿,真皮内毛细血管周围慢性炎细胞浸润,局部脂肪小叶内可见慢性炎细胞浸润,以淋巴细胞浸润为主;皮损组织EBER原位杂交(+)。免疫组化示:单一核细胞CD3,CD8,CD45RO,CD5,粒酶B均(+),CD4,CD20,CD56均(-);Ki-67(+,15%)。诊断:非典型种痘样水疱病。更多还原

【Abstract】 A case of atypical hydroa vacciniforme was reported. A 16- year- old female patient,presented with erythema,blisters,cruste and scars on face,trunk and extremities as well as with itching and pain for 7 years repeatedly,aggravating after exposure to sunlight. Histopathological examination revealed that epidermal hyperplasia. Local tissue degeneration and necrosis were observed as well as with the dermal papilla edema and dermal chronic inflammatory cell infiltration around the capillaries. Local fat lobules were infiltrated by chronic inflammatory cell,mainly lymphocyte. The skin lesions of EBER in situ hybridization was positive.Immunohistochemistry examination showed that the mononuclear cells were positive for CD3,CD8,CD45 RO,CD5 and granzyme B,but negative for CD4,CD20 and CD56,with 15% of Ki- 67 positive cells. The diagnosis was atypical hydroa vacciniforme.更多还原
