爪型肋骨接骨板内固定治疗多发性肋骨骨折的效果

【摘要】 目的探讨应用爪型肋骨接骨板内固定治疗多发性肋骨骨折患者的临床效果。方法选择2012年1月~2014年10月肇庆市第一人民医院诊治的多发性肋骨骨折患者95例,其中采用爪型肋骨接骨板内固定治疗的患者45例为观察组,采用传统保守治疗方法的50例患者为对照组。观察两组的临床疗效、不良反应发生率、胸痛持续时间、发热天数、机械通气时间、住院时间及骨折愈合时间;应用视觉模拟评分法(VAS)评估患者的疼痛状况;检测治疗前及术后1周患者二氧化碳分压(PaO2)和血氧分压(PaCO2)。结果观察组患者术后VAS评分、胸痛持续时间、发热天数、机械通气时间、住院时间及骨折愈合时间均低于对照组,差异有统计学意义(P<0.05)。观察组患者3个月内治愈率(68.89%)高于对照组(46.00%),观察组治疗后不良反应发生率(4.44%)低于对照组(18.00%),差异有统计学意义(P<0.05)。观察组治疗后PaO2[(82.63±2.06)mm Hg]高于对照组[(77.97±1.94)mm Hg],观察组治疗后PaCO2[(41.24±0.92)mm Hg]低于对照组[(45.63±0.76)mm Hg],差异有统计学意义(P<0.05)。结论应用爪型肋骨接骨板内固定治疗多发性肋骨骨折临床效果较好,能够降低并发症发生率,显著改善患者术后呼吸功能,缩短骨折愈合时间。

【Abstract】 Objective To discuss the clinic effect of claw type rib bone plate internal fixation in the treatment of multiple ribs fracture. Methods From January 2012 to October 2014, in the First People’s Hospital of Zhaoqing City, 95 patients with multiple ribs fracture were selected, patients applied claw type rib bone plate internal fixation treatment were as the observation group, 50 patients given the traditional conservative treatment were as the control group. The clinical effect, adverse reaction incidence rate, the duration time of chest pain, fever time, mechanical ventilation time,hospitalization time and fracture healing time of two groups were observed. Pain state of patients was evaluated by applying visual analogue scale(VAS); carbon dioxide(Pa O2) and partial pressure of oxygen(Pa CO2) of patients before and after treatment were detected. Results VAS score, the duration time of chest pain, fever time, mechanical ventilation time, hospitalization time and fracture healing time were in the observation group were lower than those of the control group, the differences were statistically significant(P < 0.05). The cure rate of observation group in three months(68.89%) was higher than that of the control group(46.00%), the total adverse reaction of the observation group after treatment(4.44%) was lower than that of the control group(18.00%), the differences were statistically significant(P < 0.05);Pa O2 of patinets in the observation group after treatment [(82.63±2.06) mm Hg] was higher than that in the control group[(77.97±1.94) mm Hg], Pa O2 in the observation group after treatment [(41.24 ±0.92) mm Hg] was lower than that of the control group [(45.63±0.76) mm Hg], the differences were statistically significant(P < 0.05). Conclusion Application of claw type rib bone plate fixation in the treatment of multiple rib fractures has a better clinical curative effect, and it can reduce the rate of complications, significantly improve the respiratory function of patients, shorten the time of fracture healing.
