175株鲍曼不动杆菌耐药性分析

【摘要】 目的探讨鲍曼不动杆菌临床分布及耐药情况,为临床防治提供参考依据。方法收集并回顾性分析哈尔滨医科大学附属第五医院2012年3月~2014年10月临床分离的鲍曼不动杆菌175株,用法国生物梅里埃公司的API 20NE试条、VITEK32鉴定系统对细菌进行鉴定,用琼脂纸片扩散法进行药敏试验。结果 175株鲍曼不动杆菌主要分布在重症监护室(ICU)、神经外科和呼吸内科。鲍曼不动杆菌对阿莫西林/棒酸、头孢西丁、替卡西林/棒酸的耐药率高于90%。对头孢哌酮/舒巴坦、米诺环素耐药率最低,分别为10.29%和20.57%。结论医院感染鲍曼不动杆菌主要来源为痰标本且多分布在ICU,有多重耐药性且呈逐年上升趋势。应加强细菌耐药性监测并根据药敏结果合理选用抗菌药物治疗,为医院感染控制提供科学依据。

【Abstract】 Objective To investigate clinical distribution and antimicmbial resistance of Acinetobacter baumannii, so as to provide reference for clinical prevention and treatment. Methods 175 strains of Acinetobacter baumannii isolated in the Fifth Affiliated Hospital of Harbin Medical University from March 2012 to October 2014 were collected and retrospectively analyzed. The identification of the bacteria was tested by French bio Merieux API 20 NE and VITEK32.Antimicmbial susceptibility tests were performed by Kirby-Bauer method. Results 175 strains of Acinetobacter baumannii mainly were distributed among intensive care unit(ICU), neurosurgery department and respiratory medicine department.The antimicrobial resistant rates of Acinetobacter baumannii to Amoxicillin/Clavulanic Acid, Cefoxitin and Ticarcillin/Clavulanic Acid were higher than 90%. The rates of Acinetobacter baumannii to Cefoperazone/Sulbactam and Minocycline were the lowest,accounting for 10.29% and 20.57% respectively. Conclusion The specimens of Acinetobacter baumannii in hospital are mainly isolated from the sputum and distributed in the ICU. The strains have multiple resistance to drugs and the numbers increase year by year. It is important to strengthen the monitoring antimicrobial resistance and the antibiotics should be used reasonably according to the antimicrobial susceptibility test in the treatment, in order to provide scientific basis for the control of hospital infection.
