不同分子亚型青年乳腺癌表皮生长因子受体表达及其临床病理分析

【摘要】 目的探讨不同分子亚型青年乳腺癌表皮生长因子受体(EGFR)表达情况及其与临床病理的关系。方法收集广东省江门市中心医院2003年5月~2014年7月经手术切除及病理证实的青年(年龄≤35岁)浸润性乳腺癌182例标本,应用Envision二步法检测肿瘤组织中雌激素受体(ER)、孕激素受体(PR)、人表皮生长因子受体2(HER2)及EGFR表达情况,分析EGFR在不同分子亚型青年乳腺癌的表达情况及其与临床病理的关系。结果 182例青年乳腺癌中Luminal A型、Luminal B型、HER2过表达型和Basal-like型EGFR的阳性率分别为43.5%(48/108)、22.2%(2/9)、93.6%(44/47)和73.1%(19/26);不同分子亚型青年乳腺癌中EGFR阳性率差异有统计学意义(χ2=35.154,P<0.05),其中HER2过表达型EGFR阳性表达率最高。不同分子亚型青年乳腺癌的EGFR阳性表达与肿瘤大小和组织学分级等无关(P>0.05),但与TNM分期、淋巴结转移数及远处转移有关(P<0.05)。患者TNM分期和HER2表达是影响患者预后的独立因素(相对危险度分别为6.013、2.612,P<0.05)。结论不同分子亚型青年乳腺癌中EGFR阳性表达率不同,不同亚型青年乳腺癌EGFR阳性表达与TNM分期和HER2表达有关,且两者为影响患者预后的独立因素。

【Abstract】 Objective To study the expression of EGFR in different molecular subtypes younger breast cancer and the relationship between it and clinicopathological characteristics. Methods From May 2003 to July 2014, in Jiangmen Central Hospital, the 182 samples from younger breast cancer(age ≤35 years) by surgery and pathology confirmed were selected, expression of ER, PR, HER2 and EGFR were detected by Envision two-step immunohistochemisty.EGFR in different molecular subtypes younger breast cancer and relationsip between it and clinicopathology were analyzed. Results In the 182 samples of younger breast cancer, the rate of the expression of EGFR in Luminal A subtypes,Luminal B subtypes, HER2 over expression subtypes and Basal-like subtypes were 43.5%, 22.2%, 93.6% and 73.1%respectively; the rate of EGFR positive expression in different molecular subtypes were compared, the difference was statistically significant(P < 0.05), and that in HER2 over expression subtypes was the highest. EGFR positive expression in different molecular subtypes was unrelated with the size of tumor, the type of histology(P > 0.05), but it was related with TNM stage and expression of HER2(P < 0.05). TNM stage and expression of HER2 were patients prognostic independent factor(RR= 6.013, 2.612, P < 0.05). Conclusion EGFR positive expression in different molecular subtypes of younger breast cancer were different. EGFR positive expression in different molecular subtypes of younger breast cancer is related with TNM stage and expression of HER2, and they are patients prognostic independent factor.
