机械通气对肺循环影响的研究进展

【摘要】 机械通气对心肺功能患者的治疗起重要作用,正压通气对于胸腔压力的改变必然对肺循环产生影响,从而对氧合及血流动力学产生变化。肺循环作为心肺相互作用的中心场所,是机械通气治疗过程中关注的重点。正压通气时呼吸运动方式的改变、平台压和呼气末正压的变化以及对肺泡毛细血管的损伤和神经-体液调节的影响均可导致肺循环的改变。临床上需根据患者心肺功能及病情变化选择呼吸模式及参数,以降低正压通气对心肺循环的负面影响。

【Abstract】 Mechanical ventilation plays an important role in treating patients of cardiopulmonary function disorders,the change of chest pressure by positive pressure ventilation has inevitably influenced the pulmonary circulation,thus changed the oxygenation and hemodynamic. As the cardiopulmonary interaction center,pulmonary circulation was more focused in the process of mechanical ventilation therapy. All aspects of the comprehensive factors led to the change of the pulmonary circulation,including the change of respiratory movement patterns,the setting of platform and positive end-expiratory pressure,the injury to the pulmonary capillaries and impact of neurohumoral regulation. In the clinical treatment,the breathing pattern and parameters choice should be according to patients’ cardiopulmonary function and condition,so it can reduce the negative impacts of positive pressure ventilation on cardiovascular and pulmonary circulation.
