慢性肺心病患者检测血浆N末端脑钠肽前体及D-二聚体水平的临床意义

【摘要】 目的探讨血浆N末端脑钠肽前体(NT-Pro BNP)及D-二聚体在慢性肺源性心脏病(肺心病)患者检测中的临床价值。方法收集2010年2月至2012年12月湖北省江陵县人民医院住院的慢性肺心病患者54例(A组),检测A组治疗前与治疗后2周血浆NT-Pro BNP、D-二聚体、动脉血二氧化碳分压(Pa CO2)及动脉血氧分压(Pa O2)。并选择20例健康者作为正常对照组(B组)。结果A组治疗前NT-Pro BNP、D-二聚体、Pa CO2水平显著高于B组(P<0.01),A组治疗后以上指标均高于治疗前,但仍低于对照组(P<0.01);A组治疗前Pa O2低于B组(P<0.01),A组治疗后高于治疗前但仍低于B组(P<0.01)。A组治疗前后NT-pro BNP与D-二聚体与Pa CO2呈显著正相关(P<0.05),与Pa O2呈显著负相关(P<0.05)。结论对慢性肺心病患者进行NT-Pro BNP与D-二聚体水平监测,有助于及时采取有效的防治措施,以便控制病情的发展,改善疾病的预后。临床还需改善患者缺氧状况、通气及适当的抗凝治疗,以便更好地控制病情。

【Abstract】 Objective To investigate the diagnostic value of plasma N-terminal pro-brain natriuretic peptide( NT-Pro BNP) and D-dimer levels in patients with chronic pulmonary heart disease. Methods A total of54 patients with chronic pulmonary heart disease treated in Jiangling People’ s Hospital from Feb. 2010 to Dec. 2012 were selected as group A. Another 20 healthy persons were selected as the control group( group B). The plasma NT-Pro BNP and D-dimer levels,as well as the arterial carbon dioxide partial pressure( Pa CO2) and arterial oxygen partial pressure( Pa O2) of the two groups before treatment and two weeks after treatment were compared. Results NT-Pro BNP and D- dimer levels as well as Pa CO2 of group A before treatment were significantly higher than those after treatment and those of group B( P < 0. 01),and those of group A after treatment were significantly higher than those of group B( P < 0. 01); Pa O2 of group A before treatment was lower than that after treatment and that of group B( P < 0. 01),that of group A after treatment was lower than group B( P < 0. 01). In group A,the NT-pro BNP and D-dimer levels were significantly positively correlated with Pa CO2( P < 0. 05) and significantly negatively correlated with Pa O2( P < 0. 05). Conclusion Monitoring on the NT-Pro BNP and D-dimer levels in patients with chronic pulmonary heart disease contributes to the timely and effective control of the disease and improvement of the disease prognosis. Improving hypoxic conditions,ventilation and appropriate anticoagulant therapy are also needed to control the disease in clinical practice.
