【摘要】 目的:分析点评我院6个科室用药医嘱不合理用药情况,探讨持续改进方案,提高我院临床用药水平。方法:6名临床药师分别对神经内科、消化内科、呼吸内科、心内科、ICU、肿瘤科6个科室4~9月部分不合理医嘱的改进情况进行整理和分析。结果:不合理医嘱总共455例,经过临床药师医嘱审核分析并干预,不合理医嘱在逐渐减少。结论:通过点评医嘱、实施干预、持续改进,我院6个科室用药医嘱合理率显著提高,由95.9%升至98.2%,但仍存在一些问题,临床药师积极应自动参加临床药物治疗实践,提供临床用药方案和建议,以进一步提高临床合理用药水平。更多还原

【Abstract】 Objective: Analysis review of the irrational drug use situation in our six departments, to improve the level of hospital clinical medication. Methods: Six clinical pharmacists arranged and analyzed the advice of some unreasonable order from April-September in neurology, digestive diseases, respiratory medicine, cardiology, ICU, oncology department. Results: A total of 455 cases of unreasonable orders were analyzed and intervened by clinical pharmacists, thus, unreasonable orders may be reduced. Conclusions: Through analyzing the comments on the doctor’s advice, with continuous intervention, improvement, the qualified rate of our hospital six departments medication orders increased significantly, from 95.9% to 98.2%, but there are still some problems, clinical pharmacists should automatically take an active part in clinical drug therapy practice, provide clinical medication solution and the suggestion, in order to further improve the level of clinical rational drug use.更多还原
