常规核磁共振成像与弥散成像在定量诊断骨质疏松中的对比研究
Comparison between routine MRI and diffusion-weighted MRI in quantitatively diagnosing osteoporosis
【摘要】 目的 :比较常规磁共振成像(magnetic resonance imaging,MRI)和弥散成像对骨质疏松症的定量诊断功能。方法:8名志愿者(男性4名、女性4名,平均年龄51.5岁)和8位患者(男性2例、女性6例,平均年龄71.3岁)同时接受MRI和弥散成像扫描,L2~L4共48块腰椎。在同一台1.5T磁共振扫描仪上依次进行T1加权成像(T1WI)、脂肪抑制T2-加权成像(FS-T2WI)及弥散-加权成像(DWI)扫描,测定T1WI、FS-T2WI的信噪比(SNR)和DWI的表观扩散系数值(apparent diffusion coefficient,ADC)几种定量参数,并对这些参数进行统计评估。结果:所有的磁共振参数与骨密度(bone mineral density,BMD)显著相关(T1WI:r=-0.49,FS-T2WI:r=-0.33,DWI:r=-0.21),正常和骨质疏松性椎骨之间有显著性差异(P<0.05)。当采用可行的阈值分析时,T1WI的敏感度、特异度和精确度分别为95.0%(19/20)、96.4%(27/28)、95.8%(46/48)。结论 :常规MRI,特别是T1WI,有潜力应用于骨质疏松的评估。
【Abstract】 Objective To compare routine MRI and diffusion-weighted MRI in the quantitative diagnosis of osteoporosis.Methods Eight volunteers(4 men and 4 women; mean age, 51.5 years) and eight patients(2 men and 6 women; mean age, 71.3 years) underwent both routine MRI and diffusion-weighted MRI. Results were obtained from each subject’s L2 to L4, for a total of 48 lumbar vertebrae. All MR examinations were performed on a 1.5T scanner to obtain T1-weighted imaging(T1WI), fat-suppression T2-weighted imaging(FS-T2WI), and diffusion-weighted imaging(DWI). Some quantitative MR parameters were determined and statistically assessed, including T1 WI, SNR of FS-T2 WI and apparent diffusion coefficient of DWI. Results All MR parameters significantly correlated with BMD(T1WI: r =-0.49, FS-T2WI: r =-0.33,DWI: r =-0.21), with significant differences among normal and osteoporotic vertebrae(P <0.05). When adapted as feasible threshold values, sensitivity, specificity, and accuracy of T1 WI were 95.0%(19/20), 96.4%(27/28), and 95.8%(46/48), respective ly. Conclusion Routine MRI, in particular T1 WI, has a potential for the assessment of osteoporosis.
