核磁共振成像引导经皮穿刺活检的临床应用
The clinical application of magnetic resonance imaging guided percutaneous biopsy
【摘要】 目的评价磁共振(MR)导引穿刺的可行性,以及相对于其他影像导引方式的优势。方法选取不同部位需穿刺活检的患者53例,其中男31例,女22例,年龄17~82岁,中位年龄48.3岁;肺部占位18例,椎体占位4例(胸椎1例,腰椎3例),肝脏占位13例,肾脏占位7例,盆腔占位11例(子宫直肠之间占位1例,精囊腺占位1例,前列腺9例)。采用光学导引设备与磁共振成像结合,对体内肿物进行穿刺活检。结果 53例患者在MR引导下顺利完成了穿刺活检,分别为肺脏、椎体、肝脏、肾脏及盆腔肿物。穿刺均一次成功,手术全部在局部麻醉下完成,术中患者耐受性良好,MR引导的穿刺活检均获得了足够标本,且无明显并发症发生。结论 MR具有较高软组织分辨率,可任意方位断层成像,可从多个角度引导穿刺,MR无电离辐射,故穿刺活检成功率高,风险更低。
【Abstract】 Objective To evaluate the feasibility of magnetic resonance(MR) guided biopsy,and to evaluate the advantages of it.Methods 53 patients with biopsy were enrolled,among these patients,31 male,22 female,age range from17 to 82 years old,median age was 48.3 years;pulmonary lesion of 18 cases,vertebral lesions of 4 cases(1 case of thoracic,and 3 cases of lumbar spine),hepatic lesions of 13 cases,renal space lesion of 7 cases,pelvic lesion of 11 cases(uterine rectum space of 1 case,seminal vesicle lesion of 1 case,prostate of 9 cases).Using optical guidance equipment and magnetic resonance imaging to complete the needle biopsy.Results 53 patients received successfully MR guided biopsy,identified as lung,vertebral body,liver,kidney and pelvic tumor respectively.All patients were well tolerated,and no complications occurred.Conclusion MR has high soft tissue resolution and no ionizing radiation,arbitrary slice imaging and puncture from multiple directions,so biopsy success rate is high and risk is low.
