【摘要】 目的:探讨本地区近20年感染性心内膜炎患者临床特征的变化。方法:回顾性分析1990年1月-2010年1月因感染性心内膜炎于笔者所在医院住院治疗的69例患者的临床资料。根据患者入院的时间,将患者分为较早期组和较晚期组。较早期组为1990年1月-1999年12月入院的患者;较晚期组为2000年1月-2010年1月入院的患者。所有患者的诊断标准均参照Duke诊断标准。观察两组患者临床特征的变化情况。结果:本研究中共有69例感染性心内膜炎患者。这20年最常见的病原菌为草绿色链球菌。1990年1月-1999年12月入院的患者其90 d内死亡率为3%,2000年1月-2010年1月入院的患者其90 d内死亡率为5%。90 d内复合事件(死亡和中枢神经系统并发症)方面,较晚期组的比例高于较早期组(P<0.05)。结论:近20年来本地区的感染性心内膜炎患者的病原菌并无明显变化,20世纪90年代与21世纪前10年患者的死亡率亦无明显变化。更多还原

【Abstract】 Objective:To investigate the changes of clinical features of IE(infective endocarditis) in last 20 years.Method:The clinical data of 69 patients in our hospital from January 1990 to January 2010 were retrospectively analyzed.According to the time the patients admitted to hospital,they were divided into the early group and the late group.The early group for January 1990 to December 1999 hospitalized patients;the late group in January 2000 to January 2010 hospitalized patients.All of the patients were diagnosed with reference to the Duke criteria.The changes of clinical features of the two groups were observed.Result:There were 69 patients with Infective endocarditis.The most frequent organism causing IE was Streptococcus viridians both in the last 20 years.The 90-day mortality of IE was 3% in the 1990 s and 5% in the 2000 s.Composite events of 90-day death and central nervous system disorders were significantly higher in the 2000 s compared with the 1990 s(P<0.05).Conclusion:During the past 20 years, the region of pathogenic bacteria in patients with infective endocarditis and no obvious change,in the 1990 s and the first 10 years of 21 century mortality in patients with no obvious change.更多还原
