【摘要】 目的探讨经纤支镜支气管肺泡灌洗术(BAL)在老年吸入性肺炎(AP)患者中的应用效果。方法选取我院2011年7月至2014年6月间收治的老年AP患者68例,随机分为实验组及对照组各34例,对照组患者接受常规治疗,实验组在常规治疗的基础上行纤支镜支气管肺泡灌洗术(BAL)。对比两组患者的临床疗效。结果治疗后两组患者RR、HR及Pa CO2均下降,Pa O2均上升,实验组改善较对照组更为显著(P<0.05);实验组的住院时间、感染控制窗出现时间、抗生素使用时间、机械通气时间,均显著低于对照组,两者差异具有统计学意义(P<0.05);实验组出现呕吐4例,发绀4例,鼻出血6例,均在术后24 h自行缓解,未见其他严重并发症。结论纤支镜支气管肺泡灌洗术对老年吸入性肺炎患者的临床疗效显著,且安全性高,对缓解疾病、改善预后具有重要意义,应加以推广及应用。更多还原

【Abstract】 Objective To explore the clinical effect of broncho alveolar lavage(BAL) for the treatment of elderly patients with aspiration pneumonia(AP). Methods Sixty-eight elderly patients with AP in our hospital from July 2011 to June 2014 were selected as the research objects and randomly divided into the control group and the experimental group, with 34 cases in each group. All patients were given routine therapy, while patients in the experimental group were given broncho alveolar lavage additionally. The clinical effects of two groups were observed and compared. Results After therapy, the RR, HR, and Pa CO2 of two groups decreased significantly, and Pa O2 increased significantly, but the experimental group was superior to the control group in all indicators after therapy(P <0.05). The length of hospital stay, time of infection control window appear, antibiotic usage time, duration of mechanical ventilation of experimental group were significantly shorter than those of control group(P <0.05). There were 4 cases of vomiting, 4 cases of cyanosis and 6 cases of epistaxis in experimental group, and all symptoms were alleviate by oneself after 24 h. Conclusions Broncho alveolar lavage(BAL) has significant effect and high safety for the treatment of elderly patients with aspiration pneumonia. It has important significance for relieving the disease and improving prognosis.更多还原
