【摘要】 目的观察腔内激光治疗下肢静脉曲张时,未高位结扎主干(单纯穿刺大隐静脉主干)与联合高位结扎主干及其分支对术后的疗效及复发率的影响。方法将50例单下肢大隐静脉曲张患者随机分为2组。未结扎组采用大隐静脉主干单纯穿刺激光灼闭,未高位结扎主干;结扎组采用大隐静脉主干穿刺激光灼闭联合高位结扎主干及其分支。2组患者小腿有明显曲张的浅静脉分支或静脉团均予激光烧灼或点状剥脱处理。2组患者术后均予弹力袜支持3~6月。2组患者均随访24个月,比较2组手术时间、住院时间、疼痛程度、误伤深静脉发生率以及24个月2组患者患肢静脉曲张复发率及大隐静脉主干再通率的差别。结果 2组患者的住院时间、误伤深静脉发生率、24个月的复发率及大隐静脉主干再通率无显著差异。未结扎组的手术时间比结扎组明显缩短,术后疼痛程度明显减轻。结论腔内激光治疗大隐静脉曲张时可以不联合高位结扎大隐静脉主干,且手术时间更短,疼痛程度更轻。更多还原

【Abstract】 Objective To observe the efficacy and postoperative recurrence rate of endovenous laser treatment( EVLT) for great saphenous varicose veins with high ligation of main trunk or without high ligation of main trunk and its branches. Methods A total of 50 varicose GSV patients were randomly divided into two groups. Ligation group was treated with endovenous laser treatment without high ligation of trunk,while non ligation group was treated with endovenous laser treatment with high ligation of main trunk and its branches. All crural varicose veins in two groups were also treated with laser cauterization or punctuate stripping process. All the patients were given stretch socks for 3 to 6 months. All patients were followed up for 24 months. The operating time,hospital stay,pain degree,injury incidence of deep vein and recurrence rate of lower limb varicose veins and recanalization rate of saphenous vein were compared between two groups. Results There were no significant differences in hospital stay,injury incidence of deep vein and the rate of great saphenous vein or recurrence and saphenous vein recanalization between two groups,but the operating time and postoperative pain of the non ligation group were shorter and lighter than those of the ligation group. Conclusion EVLT for great saphenous varicose veins without high ligation of main trunk can shorten operating time and reduce pain degree.更多还原
