儿童过敏性紫癜复发的相关因素分析

【摘要】 目的分析与儿童过敏性紫癜(HSP)复发相关的临床因素,为临床干预提供理论依据。方法收集330例HSP患儿,门诊随访2年,比较HSP复发患儿(复发组)与无复发患儿(无复发组)的性别、年龄、感染情况、预防用药情况、饮食情况、运动情况、嗜酸性粒细胞计数等临床资料,行多因素Logistic回归分析HSP复发的相关因素。结果门诊随访2年期间,HSP复发135例(40.9%),无复发195例(59.1%)。复发组患儿的感染率、预防用药率、饮食控制、剧烈运动限制率、嗜酸性粒细胞数目与无复发组比较差异有统计学意义(P<0.05)。2组患儿的性别、激素应用率比较差异无统计学意义(P>0.05)。多因素Logistic回归显示,治愈后有呼吸道感染、未予任何药物预防、未控制饮食、嗜酸性粒细胞持续升高是HSP复发危险因素(P均<0.05)。结论 HSP容易复发,HSP治愈后应行药物预防、避免感染、控制饮食、限制剧烈运动,降低复发率。

【Abstract】 Objective To explore the risk factors of recurrent Henoch-Schonlein purpura( HSP),and provide the theoretical basis for clinical intervention of HSP. Methods A total of 330 children diagnosed with HSP were followed up at out-patient for two years and assigned into two groups based upon the recurrence of HSP. The patients’ age,gender,situation of infection and use of preventive medicine,diet,exercise and eosinophil count were statistically compared between recurrent and non-recurrent groups. The risk factors of recurrent HSP were statistically analyzed by Logistic regression analysis. Results During 2-year out-patient follow up,135 children( 40. 9%) presented with recurrent HSP and 195( 59. 1%) had no recurrence. The infection rate,use of preventive medicine,diet control,exercise limitation and eosinophil count significantly differed between recurrent and non-recurrent children( all P < 0. 05),whereas no statistical significance was observed in terms of gender and hormone use( both P > 0. 05). Conclusions HSP has a high recurrent rate. Use of preventive medicine,control of diet and infection and exercise limitation are recommended to reduce the recurrence rate of HSP.
