同型半胱氨酸与H型高血压患者血小板活化的相关性及依那普利叶酸片的干预研究
Elevated homocysteine is associated with increased platelet activation in H-type hypertensive patients:effects of Enalapril/folic acid administration
【摘要】 目的 研究H型高血压患者血同型半胱氨酸水平与其体内血小板活化因子(CD41、CD62P)的关系,以及依那普利叶酸片对H型高血压患者血同型半胱氨酸和血小板活化因子(CD41、CD62P)的影响。方法 选取轻中度H型高血压患者200例,将其随机分为试验组和对照组各100例。试验组给予口服依那普利叶酸片,对照组给予口服依那普利,疗程均为3个月。治疗前后分别测量患者血压、超敏C反应蛋白、同型半胱氨酸、血小板活化因子(CD41、CD62P)。结果 治疗3个月后,与对照组相比,试验组平均动脉压[(80.2±6.6)mm Hg比(81.6±8.1)mm Hg]、超敏C反应蛋白[(3.1±1.3)mg/L.比(3.7±1.5)mg/L]、血同型半胱氨酸[(14.1±3.1)μmol/L比(20.2±44)μmol/L]及血小板活化因子CD41[(47.5±4.1)%比(54.5±3.9)]、CD62P[(18.5±3.1)%比(23.6±3.8)%]均下降,差异有统计学意义(P<0.05或P<0.01)。相关分析显示,血同型半胱氨酸水平与H型高血压患者体内血小板活化因子呈正相关(CD41 r=0.61,P<005;CD62P r=0.55,P<005)。结论 高同型半胱氨酸可增加H型高血压患者体内血小板活化。依那普利叶酸片在降血压和降血浆同型半胱氨酸浓度的同时还可降低H型高血压患者炎症与血小板活化。与单纯依那普利治疗相比,依那普利叶酸片可更有效减少H型高血压患者心血管发病风险。
【Abstract】 Objective To study the relationship between serum homocysteine and platelet activator(CD62P,CD41),and the influence of Enalapril/folic acid on platelet activator(CD62P,CD41) in H-type hypertensive patients.Methods 200 patients with mild to moderate hypertension were enrolled and randomly divided into two groups(the treatment group and the control group),100 for each group respectively.Patients in the treatment group were treated with Enalapril/folic acid for 3 months,while the control group with Enalapril.Blood pressure,high sensitive c-reactive protein,homocyseine,platelet activator(CD62P、CD41) were measured in all patients pre-and post-3 month’s therapy.Results Mean arterial blood pressure(80.2±6.6)mm Hg vs(81.6±8.1)mmHg,high sensitive c-reactive protein(3.1 ± 1.3)mg/L vs(3.7±1.5)mg/L,homocysteine(14.1±3.1)μmol/L vs(20.2±4.4)μmol/L,platelet activator CD41(47.5±4.1)%vs(54.5±3.9) and CD62P(18.5±3.1)%vs(23.6±3.8)%in the treatment group were all reduced significantly as compared with that in the control group(P<0.05 or P<0.01) and homocysteine was positively correlated to serum platelet activator in H-type hypertensive patients(CD41 r=0.61,P<0.05,CD62 P r=0.55,P<0.05).Conclusion Homocysteine can increase platelet activation in H-type hypertensive patient,Enalapril/folic acid is not only effective in controling of blood pressure but also effective in reducing of inflammation and platelet activation in H-type hypertensive patient,Enalapril/folic acid is more effective in lowing cardiovascular risk as compared with Enalapril in H-type hypertensive patient.
