冠心病患者微量白蛋白尿与血管内皮功能的相关性研究

【摘要】 目的 探究冠心病(CHD)患者微量白蛋白尿与血管内皮功能的相关性。方法 选取2013年6月至2014年10月我院住院CHD患者150例,依据患者Gensini积分四分位间距将患者分为4组:A组(3~19分)36例、B组(19~34分)39例、C组(34~58分)35例和D组(58~150分)40例;同时选择同期住院,行冠脉造影结果 为阴性的40例患者为对照组。采用放射免疫分析法测定患者血管内皮素-1(ET-1)、血管紧张素Ⅱ(AngⅡ)的浓度,酶联免疫吸附法测定血管内皮生长因子(VEGF)、超敏C反应蛋白(hs-CRP)的浓度,采用免疫透射比浊法和氧化酶法测定尿微量白蛋白和尿肌酐的浓度,并计算其比值白蛋白/肌酐浓度值(UACR)。结果 五组间比较显示,UACR、ET-1、AngⅡ、VEGF及hs-CRP值不完全相等,差异有统计学意义(F=87.313、34.312、37.931、354.215、63.214,P<0.05);两两比较显示,D组各指标均高于其他四组,C组高于A、B组及对照组,B组高于A组和对照组,差异均有统计学意义(P<0.05);A组与对照组比较,未见统计学差异(P>0.05)。UACR与AngⅡ、ET-1、VEGF、hs-CRP均呈正相关,相关系数分别为(r=0.312,0.209,0.517,0.414;P<0.05)。结论 AngⅡ、ET-1、VEGF、hs-CRP可反映血管内皮功能,UACR水平可用于对CHD患者血管内皮功能的评估。

【Abstract】 Objective To explore the relationship between microalbuminuria and vascular endothelial function in patients with coronary heart disease(CHD).Methods 150 cases of patients were selected with CHD and divided into 4 groups with the categorical threshold of Gensini score by quartile values:A group(3-19) with36 cases,B group(19-34) with 39,C group(34-58) with 35 and D group(58-150) with 40.The levels of ET-1and AngⅡ were measured by radioimmunoassay,VEGF and hs-CRP by enzyme-linked immunosorbnent assay,urinary albumin by immunity transmission turbidity,and creatinine by oxidase method,then computed the ratio of urinary albumin to creatinine.Results The level of UACR,ET-1,Ang Ⅱ,VEGF and hs-CRP were significant difference among the five groups(F=87.313,34.312,37.931,354.215,63.214,P<0.05).The value of AngⅡ,ET-1,VEGF,hs-CRP among the four Groups,D group were higher than other four groups,C group higher than that of A and B and the control group,B group was higher than A group and the control group,the differences were statistically significant(P<0.05).There was no significant difference between group A and control group(P>0.05).There was positive corelation between UACR and ET-1,AngⅡ,VEGF,hs-CRP and the correlation coefficient were(r=0.312,0.312,0.517,0.414,P<0.05)respectively.Conclusion The ET-1,AngⅡ,VEGF and hs-CRP indexes are marks of endothelial function,the level of UACR can be used for the evaluation of endothelial function in patients with CHD.
