【摘要】 目的系统评价硫酸镁在预防冠状动脉旁路移植术(CABG)后心房颤动的临床疗效。方法计算机检索The Cochrane Library(2014第4期)、Pub Med、EMbase、CBM、CNKI、Wan Fang Data和VIP,收集有关硫酸镁预防CABG后心房颤动相关的随机对照试验(RCT),检索时限均为从建库至2014年9月,由2位评价者根据纳入、排除标准独立选择文献、提取资料和评价纳入研究的方法学质量后,采用Rev Man5.2软件进行Meta分析。结果最终纳入7个RCTs,共计1030例患者,其中硫酸镁组(试验组)513例,安慰剂组(对照组)517例。Meta分析结果显示:与安慰剂组相比,硫酸镁能显著降低CABG术后心房颤动的发生率[RR=0.56,95%CI(0.40,0.78),P=0.0006]。结论围术期预防性应用注射用硫酸镁能显著降低CABG术后心房颤动的发生率,值得临床借鉴,受纳入研究质量和数量所限,上述结论仍需开展更多高质量的RCT加以验证。更多还原

【Abstract】 Objective To systematically review the efficacy of magnesium sulfate on the prevention of atrial fibrillation after coronary artery bypass grafting( CABG). Methods Databases including The Cochrane Library( Issue 4,2014),Pub Med,EMbase,CBM,CNKI,Wan Fang Data and VIP were searched from the establishment of date to September 2014,relevant randomized controlled trials( RCT) related to magnesium sulfate preventing atrial fibrillation after CABG were collected. After two reviewers selected literature,extracted data and evaluated the methodological quality of the included studies based on the inclusion and exclusion criteria independently,Meta-analysis was performed by using Rev Man 5. 2 software. Results A total of seven RCTs involving 1030 cases were included,of which 513 cases were of the magnesium sulfate group( experimental group),517 cases were of the placebo group( control group). The results of meta-analysis showed that compared with placebo group,magnesium sulfate significantly reduced the incidence of atrial fibrillation after CABG [RR = 0. 56,95% CI( 0. 40,0. 78),P = 0. 0006]. Conclusion Magnesium sulfate can significantly reduce the incidence of atrial fibrillation after CABG,it is worth referencing,due to the quality and quantity of the included studies limited,the conclusion needs to be verified by carrying out more high-quality RCTs.更多还原
