夜班高血压患者降压效果与ABPM使用依从性的相关性
Relationship between antihypertensive efficacy and compliance with ABPM use in hypertensive patients on night shift
【摘要】 目的分析夜班高血压患者降压效果与动态血压监测仪(ABPM)使用依从性的相关性。方法选取我院2012年8月~2014年5月321例夜班高血压患者为研究对象,通过问卷调查其ABPM使用及认知情况,检测其血压控制水平,并进行ABPM使用依从性的单因素分析。结果 321例中,共222例依从性合格,合格率为69.16%;血压监测认知和使用频率好患者的血压控制率均显著高于一般和差的患者(P<0.05),一般患者又显著高于差的患者(P<0.05)。单因素分析显示,性别、吸烟、文化水平与夜班高血压患者ABPM依从性无明显关联,而年龄、白天睡眠时间及治疗费用支付方式则为夜班高血压患者ABPM的影响因素。结论年龄、白天睡眠时间及治疗费用支付方式是夜班患者ABPM使用依从性的主要影响因素,临床需对ABPM使用依从性较低的患者开展健康宣教,以提高其对ABPM使用的依从性,从而提高血压控制效果。
【Abstract】 Objective To analyze the relationship between antihypertensive efficacy and compliance with ambulatory blood pressure monitor(ABPM) in hypertensive patients on night shift. Methods 321 hypertensive patients on night shift who were admitted into the hospital between August 2012 and May 2014 were selected as the research objects. The use and cognition of ABPM were investigated through questionnaires. Their blood pressure control levels were detected. Single factor analysis was carried out to study the compliance with ABPM use. Results Among the 321 cases, the compliance in 222 ones was up to standard, and the qualification rate was 69.16%. The rate of blood pressure control in patients who had a good knowledge of ABPM was significantly higher than that of patients who had general and poor cognition degrees of the knowledge about ABPM(P < 0.05). And that of the patients with general cognition was higher than that of those with poor cognition(P < 0.05). The single factor analysis showed that gender, smoking, and educational levels had no significant relationship with the compliance with ABPM in hypertensive patients on night shift, while age,daytime sleep time, and the mode of payment for the treatment costs were the influencing factors of ABPM. Conclusion Age, daytime sleep time, and the mode of payment for treatment costs are the main influencing factors of the compliance with ABPM use in patients on night shift. In clinic, health education needs to be implemented among the patients who have relatively lower compliance with ABPM use in order to increase the compliance and improve the effects of blood pressure control.
