新疆维吾尔、汉族帕金森病患者与25-羟基维生素D3浓度及骨密度相关性分析

【摘要】 目的探讨新疆维吾尔、汉族帕金森病(PD)患者25-羟基维生素D3及骨密度的差异及其与PD的相关性。方法选择108例原发性帕金森病患者(PD组),其中汉族78例,维吾尔族30例,选择108例健康体检者为对照组,其中汉族78例,维吾尔族30例。采用高效液相色谱法检测血清中25-羟基维生素D3的含量,用双能X线骨密度测量法检测骨密度,分析维吾尔、汉族患者25-羟基维生素D3值及骨密度值是否存在差异及其与PD的关系。结果 PD组患者25-羟基维生素D3与骨密度(BMD)值均低于对照组(P<0.05)。女性PD患者25-羟基维生素D3与骨密度(BMD)值均低于对照组(P<0.05);男性PD组与对照组25-羟基维生素D3及骨密度差异无统计学意义(P>0.05)。汉族PD患者25-羟基维生素D3与BMD值低于对照组汉族(P<0.05);维吾尔族PD患者BMD值低于对照组维吾尔族(P<0.05),维吾尔族PD组与对照组25-羟基维生素D3值差异无统计学意义(P>0.05)。PD组男性患者的BMD值均高于女性(P<0.05),汉族PD患者25-羟基维生素D3值与BMD值均低于维吾尔族患者(P<0.05)。将与PD相关的5个因素进行logistics回归多因素分析结果显示:年龄差异有统计学意义[P=0.019,OR=0.959,95%CI(0.927~0.993)],25-羟基维生素D3[P=0.004,OR=0.923,95%CI(0.874~0.975)]、BMD[P=0.016,OR=0.017,95%CI(0.001~0.469)]差异有统计学意义;采用逐层向前分析显示:25-羟基维生素D3与PD存在关联,即25-羟基维生素D3每增加1个单位,PD的发生风险就降低0.062。结论新疆维吾尔族PD患者25-羟基维生素D3值及BMD值高于汉族PD患者;25-羟基维生素D3可能是PD的保护因素。

【Abstract】 Objective To investigate whether vitamin D3(VD3)and bone mineral density of the Xinjiang Uygur and Han nationality are different and related to Parkinson′s disease(PD).Methods Using HPLC to detect the content of serum 25 hydroxyl VD3in the two groups 108 patients with primary PD(78Han patients and 30 Uygur patients)and 108 healthy population(same ratio of Han and Uygur to patients group)and dual energy X-ray bone density to investigate bone density of,and according to the VD3 value and bone mineral density,analyze differences and relationship between Uygur and Han patients with PD.Results The VD3 and BMD value in PD group are lower than in control group(P <0.05).The VD3 and BMD value of female PD patients were lower than that of the control group(P <0.05),and there is no difference between male PD and control group(P >0.05).The VD3 and BMD value of Han nationality with PD are lower than in control group(P <0.05).In Uygur nationality patients with PD,the BMD value is lower than in control group(P <0.05)and there is no difference in VD3(P >0.05).The BMD value of male patients with PD are higher than female(P <0.05),the VD3 and BMD value of Han patients with PD are lower than the Uygur patients(P <0.05).The results of the five factors related to PD by logistic univariate regression showed that there is significant difference in age(P=0.019,OR =0.959),VD3(P=0.004,OR =0.923)and the BMD(P=0.016,OR =0.017);the results of using layer by layer forward analysis showed that there is significant correlation between VD3 and PD,if VD3 increases each additional1 unit,the risk of PD could be reduced 0.062.Conclusion The VD3 and BMD value of Han patients with PD are higher than those of Uygur patients with PD in Xinjiang and 25 hydroxyl VD3is the protective factor for PD.
