急诊介入治疗前后AMI患者P选择素和血小板膜糖蛋白GPⅡb/Ⅲa复合物表达研究

【摘要】 目的观察急性心肌梗死(Acute myocardial infarction,AMI)患者急诊经皮冠状动脉介入治疗(简称介入治疗)前后血小板活化指标P选择素和血小板膜糖蛋白GPⅡb/Ⅲa复合物表达的变化,探讨介入治疗对AMI患者血小板活化的影响。方法选取疑似冠心病且冠脉造影正常的21例患者为对照组,行急诊介入治疗的AMI患者24例(AMI组),应用流式细胞技术检测对照组及AMI组患者行介入治疗前后的P选择素和血小板膜糖蛋白GPⅡb/Ⅲa复合物表达率。结果与对照组比较,AMI组患者P选择素和血小板膜糖蛋白GPⅡb/Ⅲa复合物表达率均明显升高,差异有统计学意义(P<0.05);介入治疗后3dAMI组患者P选择素和血小板膜糖蛋白GPⅡb/Ⅲa复合物表达率较术前下降,但与对照组比较仍有较高水平表达,差异有统计学意义(P均<0.05)。结论AMI患者急诊介入治疗前后体内血小板均处于高度活化状态,AMI患者术后抗血小板聚集治疗极其重要。

【Abstract】 Objective To Observe the changes of P-selectin(CD62P)and platelet membrane glycoprotein of GPⅡb/Ⅲa(PAC-1)in patients with Acute myocardial infarction(AMI)before and after Emergency percutaneous coronary intervention,and to investigate the influnce of platelet activation in AMI patients with intervention therapy.Methods A total of 21 patients that suspected CHD and normal coronary angiography were served as control group,and 24 AMI patients were primary percutaneous coronary intervention.Measured the levels of P-selectin(CD62P)and platelet membrane glycoprotein of GPⅡb/Ⅲa(PAC-1)by flow cytometry in AMI patients before and after interventional therapy,and then compared with normal control group.Results The expression rates of CD62 Pand PAC-1in AMI patients were significantly higher than those in controls(P <0.05),but the CD62 Pand PAC-1expression were significantly lower after 3days interventional therapy,but still higher than controls(P<0.05).Conclusion Platelets in AMI patients before and after emergency percutaneous coronary intervention were in a highly activated state,so it is extremely important to antiplatelet aggregation therapy for the AMI patients after interventional therapy.
