肾功能异常患者MSCV与MCV比值的变化及临床意义
Clinical significance of MSCV and MCV ratio changes in patients with abnormal renal function
【摘要】 目的观察肾功能异常患者球形细胞平均体积(MSCV)和平均红细胞体积(MCV)的比值变化,探讨MSCV/MCV与Cr、urea及Hb的相关性及疾病治疗过程MSCV/MCV变化的临床意义。方法用Uni Cel Dx H800 Coulter血液分析仪、AU5821全自动生化分析仪分别检测80例肾功能异常患者、80例非肾功能异常贫血患者与80例健康对照者MSCV、MCV、Hb、Cr、urea,用Pearson相关分析MSCV/MCV与Cr、urea和Hb的相关性,调查18例肾功能异常患者治疗期间urea与Hb及MSCV/MCV的变化。结果肾功能异常组MSCV/MCV为1.007±0.06,明显低于健康对照组MSCV/MCV(1.047±0.03),差异有统计学意义(t=5.885,P<0.01),也低于贫血组MSCV/MCV(1.042±0.04,t=4.545,P<0.01)。MSCV/MCV<1发生率,肾功能异常组为35.0%(28/80),贫血组为11.25%(9/80),健康对照组为2.5%(2/80),肾功能异常组与其他2组分别比较差异均有统计学意义(χ2=12.69、27.73,P<0.01)。MSCV/MCV与urea呈负相关(r=-0.5836 P<0.01),与Cr、Hb没有相关性(P>0.05)。肾功能异常患者治疗期间MSCV/MCV随着urea的下降而上升。结论肾功能异常患者易出现MSCV<MCV的现象,MSCV/MCV与urea呈负相关。肾功能异常患者治疗过程同时观察MSCV/MCV与urea的变化,对了解治疗效果有参考意义。
【Abstract】 Objective To investigate the MSCV and MCV ratio changes in patients with abnormal renal function,and explore the correlation between MSCV/MCV and Cr, urea and Hb, and the clinical significance of MSCV/MCV changes in the process of the treatment of the disease. Methods Totally 80 patients with abnormal renal function, 80 anemia patients with non-abnormal renal function, and 80 healthy persons were investigated for their MSCV, MCV, Hb, Cr, and urea by Uni Cel Dx H 800 Coulter blood analyzer and AU5821 automatic biochemical analyzer. The correlations between MSCV/MCV and Cr, urea and Hb were analyzed by Pearson correlation analysis method. The changes of urea, Hb and MSCV/MCV were investigated in 18 patients with abnormal renal function during the treatment process. Results MSCV/MCV in the abnormal renal function group was 1.008 ±0.06, which was significantly lower than that in the healthy control group(1.047±0.03)(t=5.350, P < 0.01), and the anemia group(1.042±0.04)(t=4.545, P < 0.01). In MSCV/MCV<1, the incidence rates were 35.0%(28/80) in the abnormal renal function group, 11.25%(9/80) in the anemia group, and 2.5%(2/80) in the healthy control group. There were significant differences between the abnormal renal function group and other two groups(χ2=12.69, 27.73, both P < 0.01). MSCV/MCV was negatively correlated with urea(r =-0.5836, P< 0.01), but was not correlated with Cr or Hb(both P >0.05). MSCV/MCV increased with the decline of urea during the treatment process in the patients with abnormal renal function. Conclusion It is easy to find the MSCV <MCV phenomenon in the patients with abnormal renal function, and MSCV/MCV is negatively correlated with urea. During the treatment process of the patients with abnormal renal function, the observation of the changes of MSCV/MCV and urea has a significant reference to evaluate the treatment effect.
