血管性介入技术在胎盘植入致产后大出血的应用

【摘要】 目的探讨血管性介入技术应用于胎盘植入致产后大出血的临床效果。方法选取我院2011年8月~2012年8月收治的40例胎盘植入致产后大出血患者,将其随机分为研究组和对照组,研究组患者行血管性介入治疗,对照组患者行B-Lynch缝合联合病灶楔形切除术,对比两组患者的临床疗效、手术时间、术中出血量及并发症发生情况。结果研究组和对照组患者均一次性止血成功,保留了子宫,两组患者均恢复正常月经,不过研究组正常月经恢复时间显著短于对照组(p<0.05)。研究组手术时间显著少于对照组(p<0.01),术中出血量显著少于对照组(p<0.05)。两组患者均未出现严重术后并发症。结论血管性介入技术应用于胎盘植入致产后大出血的临床效果显著,可有效止血,且手术时间短、术中出血量少,值得临床推广。

【Abstract】 Objective To study the clinical effects of vascular intervention in the treatment of postpartum hemorrhage caused by placenta accreta. Methods A total of 40 postpartum hemorrhage caused by placenta accreta patients received from Aug 2011 to Aug 2012 were included in this study and randomly divided into study group and control group. Patients in study group were treated by vascular intervention and patients,and the control group were treated by B- Lynch suture joint lesions wedge resection. The clinical efficacy,operative time,blood lose volume and complications were compared between two groups. Results All the patients were hemostasis successfully and retained the uterus. The menstruation in two groups were all returned to normal. But the recovery time in study group were significantly less than that in control group( p < 0. 05). The operation time and blood lose volume in study group were significantly less than that in control group( p < 0. 01,p < 0. 05). There was no severe complication in both groups. Conclusion Vascular intervention has a great clinical effects in the treatment of postpartum hemorrhage caused by placenta accrete,which provide a better clinical effects and less operative time and blood lose,and was worth for clinical application.
