【摘要】 目的观察联合手术和单纯下鼻甲部分切除术治疗变应性鼻炎的疗效及其对白细胞介素-4(IL-4)、干扰素-γ(INF-γ)和单核细胞趋化蛋白-1(MCP-1)水平的影响。方法将79例变应性鼻炎患者根据手术方式不同分为2组,观察组34例行联合切除术,对照组45例行下鼻甲部分切除术。3个月后观察2组疗效,治疗前后症状、体征评分和IL-4、INF-γ和MCP-1水平变化。结果观察组总有效率明显高于对照组(X2=4.401,P<0.05)。2组治疗前喷嚏、流涕、鼻塞、鼻痒评分及体征评分和血清IL-4、INF-γ和MCP-1水平比较差异均无统计学意义(P均>0.05);治疗后2组喷嚏、流涕、鼻塞、鼻痒评分及体征评分和血清IL-4和MCP-1水平均较治疗前明显降低(P均<0.05),血清INF-γ水平较治疗前明显升高(P均<0.05),且观察组降低和升高水平较对照组更为明显(P均<0.05)。结论联合手术治疗变应性鼻炎疗效确切,其机制可能与恢复机体的细胞因子平衡有关。更多还原

【Abstract】 Objective It is to observe the efficacy of combined surgery and simple partial resection of inferior turbinate in patients with allergic rhinitis and their impact on the levels of interleukin-4( IL-4),interferon-γ( INF-γ) and monocyte chemoattractant protein-1( MCP-1). Methods 79 patients with allergic rhinitis were divided into two groups according to surgical methods,34 cases in observation group were treated with combined surgery and 45 cases in control group were treated with simple partial resection of inferior turbinate. The efficacy and the levels of symptom scores,IL-4,INF-γ and MCP-1 before and after treatment were observed in each group. Results The total effective rate in observation group was markedly higher than that in control group( X2= 4. 401,P < 0. 05). There were no significant differences in the scores of sneezing,runny nose,nasal congestion,nasal itching,and levels of IL-4,INF-γ and MCP-1 before treatment between the both groups( all P > 0. 05); after treatment,the scores of sneezing,runny nose,nasal congestion,nasal itching and levels of IL-4 and MCP-1 in two groups were significantly lower than those before treatment( all P < 0. 01),and the lower levels in observation group were more obvious than in control group( all P < 0. 01); and the levels of INF-γ in two groups after treatment were significantly higher than those before the treatment( all P < 0. 01),and the elevated levels were more obvious in observation group than that in control group( P < 0. 01). Conclusion The efficacy of combined surgery is obvious in patients with allergic rhinitis; the mechanism may be related to restore the body’s balance of cytokines.更多还原
