【摘要】 目的了解钦州市男性嫖客商业性行为的特点,初步评估嫖客人群感染艾滋病病毒(HIV)可能相关的危险因素。方法在钦州市的暗娼场所内,对男性嫖客开展问卷调查,获取人口学和商业性行为学等信息,采集5mL全血标本用于HIV和梅毒抗体检测,使用SPSS 18.0软件进行统计分析。结果共调查1196名嫖客,65.05%来自低档场所,74.92%的嫖客从不使用或很少使用安全套,22.07%的人在嫖娼时使用"壮阳药",79.52%的人近30天内嫖娼次数>2次,平均花费为40元/次(中位数),商业性行为方式主要是阴道性交(97.32%)和手淫(28.18%)。HIV和梅毒阳性检出率分别为1.42%(17/1196)和9.95%(119/1196)。离婚/丧偶/分居[比值比(OR)=5.552,95%可信区间(CI):1.792~17.201]、未婚(OR=5.653,95%CI:1.329~24.054)、在城郊农舍嫖娼(OR=3.333,95%CI:1.189~9.341)、使用壮阳药(OR=4.027,95%CI:1.456~11.135)和梅毒阳性(OR=9.042,95%CI:3.232~25.293),是嫖客感染HIV的危险因素。结论钦州市男性嫖客HIV和梅毒感染率较高,商业性行为具有消费低档、形式多样、频繁暴露、使用药物和缺乏防护等特点,其感染HIV的相关危险因素可能包括离婚/丧偶/分居、未婚、使用壮阳药物、梅毒阳性和在城郊低档场所嫖娼。更多还原

【Abstract】 Objective To understand the characteristics about the commercial sexual behavior of male clients in Qinzhou city and to explore the related risk factors of HIV infection.Methods Questionnaire survey was conducted to collect information of male clients who were found in the CSW places.Five ml blood sample was taken to test antibodies of HIV and syphilis.SPSS 18.0was used to analysis data.Results A total of 1196 questionaires were completed.65.05% of the male clients came from low grade places and 74.92% of the participants never or rarely used the condom when they have commercial sex behavior.22.07% of the participants said they used aphrodisiac when having sex.79.52% of them patronised prostitutes at least 2times in the last 30 days with an average cost of40 yuan each time(median).The main way of commercial sexual behavior are vaginal intercourse(97.32%)and masturbation(28.18%).The detection rate of HIV and syphilis was 1.42%(17/1196)and 9.95%(119/1196),respectively.HIV infection risk factors inclued divorced/widowed/separated(OR=5.552,95%CI:1.792-17.201),unmarried(OR=5.653,95%CI:1.329-24.054),use aphrodisiac(OR=4.027,95%CI= 1.456-11.135),positive for syphilis(OR=9.042,95%CI:3.232-25.293)and prostitution in outskirts of town(OR=3.333,95%CI:1.189-9.341).Conclusion The male clients have high infection rate of HIV and syphilis in Qinzhou.The commercial sexual behaviors of them include low consumption level,diversification,frequent exposure,use drugs and lack of protections.The high risk factors of HIV infection may be"divorced/widowed/separated","unmarried","use drugs","syphilis positive"and"prostitution in the low grade places in outskirts of town."更多还原
