心肌酶谱与肌钙蛋白I联合检测对急性心肌梗死早期诊断的价值
The Values for the Early Diagnosis of Acute Myocardial Infarct by Detection of Myocardial Enzymes and Cardiac Troponin I
【摘要】 目的探讨心肌酶谱与血清肌钙蛋白I(c Tn I)联合检测对急性心肌梗死(AMI)早期诊断的价值,提高AMI诊断正确率。方法检测120例疑似AMI患者心肌酶谱及c Tn I,并以心电图检查为金标准,分析心肌酶谱、c Tn I单独检测与联合检测对AMI早期诊断的价值。结果心电图检出AMI 107例,非AMI 13例;心肌酶谱诊断AMI的灵敏度、特异度、阳性预测值、阴性预测值分别为85.05%、76.92%、96.81%、38.46%;c Tn I诊断AMI的灵敏度、特异度、阳性预测值、阴性预测值分别为89.72%、92.31%、98.97%、52.17%;二者联合检测阳性率为88.33%,高于单独检测诊断阳性率(78.33%,80.83%)。结论 c Tn I单独检测对AMI诊断价值优于心肌酶谱,但两者联合检测诊断准确度高于单独检测,可提高AMI早期诊断正确率。
【Abstract】 Objective To investigate the value of myocardial enzyme combined cardiac troponin I(c Tn I)detection in early diagnosis of acute myocardial infarction(AMI),improving AMI diagnostic accuracy. Methods Detected the myocardial enzymes and c Tn I of 120 cases with suspected AMI, and ECG as the gold standard. Analysis the value of myocardial enzymes, c Tn I individual detection and combined detection for the early diagnosis of AMI. Results 107 cases of AMI and non-AMI 13 cases were detected by ECG,the sensitivity of myocardial enzymes diagnosis of AMI,specificity, positive predictive value, negative predictive value were 85.05%, 76.92%, 96.81%, 38.46%. By contrast,and c Tn I were 89.72%,92.31%, 98.97%,52.17%,combined detection positive rate was 88.33%,it was higher than the individual detection(78.33%,80.83%). Conclusion c Tn I individual detection is better than myocardial enzymes,but combined detection diagnostic accuracy is much higher,and it can improve the accuracy of early diagnosis of AMI.
