多体素1H-MRS对脑肿瘤、脑脓肿、脑梗死的鉴别诊断的临床思路构架
An Exploration of the Effect of Multivoxel 1H-MRS in the Differential Diagnosis of the Brain Tumor,Brain Abscess and Cerebral Infarction
【摘要】 目的:探讨多体素1H-MRS对脑肿瘤、脑脓肿、脑梗死临床诊断的应用价值。方法:回顾性分析2012年5月至2013年5月我院收治的脑部疾病患者177例作为研究对象,对所有患者的临床资料进行详细分析。结果:32例脑肿瘤患者的Cho峰高于对侧,大部分NAA峰低于对侧;大部分脑脓肿患者的NAA峰、Cho峰都低于对侧,93例脑梗死的NAA峰、Cho峰和Cr峰都明显降低,85例患者的Lac峰比较高。脑肿瘤和脑梗死的Cho/Cr、NA A/Cho、Cho/Cr-n有显著差异(P<0.05),有统计学意义;比较三组的r Cho比值,均有显著差异(P<0.05),有统计学意义。结论:采用多体素1H-MRS对脑肿瘤、脑脓肿、脑梗死等脑部疾病进行鉴别诊断,科学有效,诊断结果可靠性高,值得在临床上进一步推广应用。
【Abstract】 Objective: To study the clinical value of the multivoxel 1H-MRS in the differential diagnosis of the brain tumor, brain abscess and cerebral infarction. Methods: The clinical data of 177 patients who used to come to our hospital for the treatment of the brain diseases during the period from May 2012 to May 2013 were retrospectively analyzed. Results: The Cho summit of 32 patients with the brain tumor was higher than that of the contralateral and most of their NAA summit was lower than that of the contralateral. Both the Cho summit and the NAA summit of the most patients with the brain abscess were lower than those of the contralateral. The Cho summit, NAA summit and the Cr summit of the 93 patients with the cerebral infarction were all significantly reduced. 85 patients had a higher Lac summit. The Cho/Cr、NA A/Cho、Cho/Cr-n of the patients with the brain tumor and those of the brain abscess were remarkably different(P<0.05). It was statistically significant. The r Cho of the three groups were all significantly different(P<0.05). Therefore, it was statistically significant. Conclusion: Multivoxel1H-MRS is scientific and effective in the differential diagnosis of the brain tumor, brain abscess and cerebral infarction. Boasting the high diagnostic accuracy, it is worthy of further promotion in the clinical application.
