植入型丹参缓释剂联合髓芯减压术对兔激素性股骨头坏死治疗机制的实验研究

【摘要】 目的研究植入型丹参缓释剂联合髓芯钻孔减压术对激素性股骨头坏死的治疗作用,同时分析其治疗机理。方法应用改良Matsui的马血清加甲强龙诱导制成兔早期GONFH模型,采用氧化卤代技术制备丹参缓释剂,将27只造模成功的实验用家兔随机分为模型组、减压组和植入型丹参缓释剂联合髓芯钻孔减压术组,分别给予常规喂养,钻孔减压治疗,钻孔减压结合丹参缓释剂治疗,另取9只健康实验用家兔作为正常对照组。于造模后2、4、8周对各组实验动物股骨头组织内BMP-2及PPAR-γm RNA的表达进行检测分析。结果丹参组在治疗后2、4、8周股骨头内BMP-2的m RNA及蛋白的表达量较模型组、减压组同时间段BMP-2的表达高,与正常组相比8周时BMP-2的表达较低;PPAR-γm RNA及蛋白的表达在丹参组中其2、4、8周表达量较模型组及减压组降低,与正常组8周时PPAR-γ的m RNA及蛋白的表达相当。结论丹参缓释剂联合髓芯钻孔减压术治疗可以调节坏死股骨头内BMP-2及PPAR-γ的表达,促进局部骨质再生与修复,达到治疗骨坏死的目的。这种治疗方法有可能成为一种临床上操作简单、经济安全、疗效较好的治疗早期GONFH的新方法。

【Abstract】 Objective: To study the curing action of the implantation of controlled-releasing salvia miltiorrhizae(ICRSM) combining operation by core decompression during the period of Glucocorticoid-induced osteonecrosis of the femoral head(GONFH)healing, and analyzing the mechanism of ICRSM during GONFH healing. Methods: 27 rabbits which were produced models of rabbit GONFH using horse-serum were randomly divided into three groups as follows: the model group, the decompression group, and the ICRSM group, which were treat with no treatment, operation by core decompression, and ICRSM(made by Oxidation-halojenation technology) combining operation by core decompression, in addtion establish normal group with normal rabbits. BMP-2 and PPAR-γwere analyzed at 2th, 4th, 8th weeks after models were produced. Result: BMP-2 in the ICRSM group were higher than the model group and the decompression group at 2th, 4th, 8th weeks, and lower than normal group at 8th weeks. PPAR-γ in the ICRSM group was lower than the model group and the decompression group at 2th, 4th, 8th weeks, and similar to the normal group at 8th weeks. Conclusion: The treatment of ICRSM combining operation by core decompression can adjust the quantity of BMP-2 and PPAR-γin the babbits femoral head, accelerate the bone in GONFH healing and repairing, finally treat GONFH and can become an easy, safty and batter treatment in the clinic operation.
