【摘要】 目的探讨失代偿期肝硬化患者经脐血干细胞移植治疗后血清中细胞因子水平的变化及其临床意义。方法选取50例失代偿期肝硬化患者,经肝动脉灌注进行脐血干细胞移植治疗,采用ELISA方法检测术前及术后第8、16、24周血清中肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、成纤维细胞生长因子-β(FGF-β)水平及肝纤维化指标的变化。结果患者在脐血干细胞移植治疗后第8周就出现肝功能好转并持续改善,第16、24周较术前明显改善,差异均有统计学意义(均P<0.05);治疗后第16、24周患者血清中TNF-α、IL-6、FGF-β水平均较术前明显下降,差异均有统计学意义(均P<0.05);术后第24周肝纤维化指标亦有明显改善(P<0.05)。结论脐血干细胞移植治疗能改善失代偿期肝硬化患者的肝功能及降低血清TNF-α、IL-6、FGF-β等细胞因子水平,有望作为一种新的治疗失代偿期肝硬化的方法而得到推广。更多还原

【Abstract】 Objective To investigate the changes of cytokines in the serum of patients with decompensated liver cirrhosis after umbilical cord blood stem cell transplantation and their clinical significance.Methods A total of 50 patients with decompensated liver cirrhosis were recruited in the study and treated with umbilical cord blood stem cell transplantation by hepatic arterial infusion.The levels of TNF-α,IL-6,FGF-βand the liver fibrosis indexes in the serum were detected by means of ELISA before and 8,16,24 weeks after transplantation and the changes of these indexes were observed.Results The liver function was improved 8weeks after umbilical cord blood stem cell transplantation and was continuously improved.There was significant difference in the liver function indexes between before and 16,24 weeks after transplantation(P<0.05).The levels of TNF-α,IL-6,and FGF-βwere conspicuously decreased in the serum of patients at 16 and 24weeks with significant difference observed when compared with those before transplantation(P<0.05).Moreover,the liver fibrosis indexes were also improved significantly at 24weeks(P<0.05).Conclusion Umbilical cord blood stem cell transplantation can improve the liver function and decrease the levels of TNF-α,IL-6and FGF-βin patients with decompensated liver cirrhosis and it is expected to become an alternative treatment for decompensated liver cirrhosis and worth popularization.更多还原
