电灼止血联合皮内缝合在心脏起搏器植入术中应用观察

【摘要】 目的:观察电灼止血术联合切口皮内缝合术在永久起搏器植入术中的应用效果。方法:选择我院从2010年1月至2014年6月行永久性心脏起搏器植入术患者共152例,其中70例应用传统压迫、结扎止血术联合传统间断缝合术,归为常规止血联合间断缝合组,82例应用电灼止血术联合切口皮内缝合术,归为电灼止血联合皮内缝合组;比较两组术中止血效果、术后住院时间、切口愈合情况、囊袋血肿、感染发生率及术后6个月随访情况。结果:较之常规止血联合间断缝合组,电灼止血联合皮内缝合组可明显减少囊袋出血、血肿等并发症,缩短术中止血时间[(7.93±0.685)min比(11.61±0.916)min,P<0.01],手术切口愈合良好,切口美观,术后住院时间明显缩短[(3.17±0.667)d比(7.74±1.349)d,P<0.01]。随访6个月起搏参数与程控参数一致,无囊袋感染。结论:电灼止血术联合切口皮内缝合术在永久性心脏起搏器植入术中的应用安全、有效,手术并发症少,切口美观,可显著缩短术后住院时间,减轻患者经济负担。

【Abstract】 Objective: To observe the effects of the permanent pacemaker implantation with fulguration hemostasis and intradermic suture. Methods: 152 patients who have been implanted the permanent pacemaker from Jan 2010 to Jun 2014 were chosen. We used the normal hemostasis and interrupted suture in 70 patients,and 82 pstients with the fulguration hemostasis and intradermic suture. The hemostasis effect,length of hospital stay,time of wound healing,pocket hematoma,infection rate and 6 months follow-up were compared. Results: The patients with the fulguration hemostasis and intradermic suture had lower infections of poket bleeding and hematoma,there was the shoeter hemostasis time(( 7. 93 ± 0. 685) min vs( 11. 61 ± 0. 916) min,P < 0. 01),less hospital stay time(( 3. 17 ± 0. 667) days vs( 7. 74 ± 1. 349) days,P < 0. 01) than the normal hemostasis and interrupted suture,also with the perfect operation incision and no pocket infection after 6 months follow- up. Conclusion: Using the fulguration hemostasis and intradermic suture in permanent pacemaker implantation is safe,effective,this method has perfect incision,less complications,less hospital stay time and less medical expense.
