C-反应蛋白与降钙素原在慢性阻塞性肺疾病诊疗中的临床意义
The evaluation of C-reactive protein and procalcitonin in the diagnosis and treatment of chronic obstructive pulmonary disease
【摘要】 目的:评价降钙素原(PCT)、C-反应蛋白(CRP)水平在慢性阻塞性肺疾病(COPD)的临床意义。方法:COPD患者164例,通过检测稳定期与急性加重期患者的PCT以及CRP的含量,评价两者对于COPD急性加重的诊断意义和疗效。结果:COPD急性加重组的PCT、CRP水平与COPD组相比显著升高;疗效观察,指导治疗组和常规治疗组患者的治疗有效率差异无统计学意义;指导治疗组患者COPD急性加重发生率与常规治疗组相比发生率下降,但组间差异无统计学意义;住院时间、抗菌治疗时间、二重感染率指导治疗组较常规治疗组显著减少。结论:PCT、CRP能够作为早期诊断COPD急性加重的血清学指标,同时PCT、CRP水平用于指导COPD的临床治疗缩短了患者的住院时间,显著降低了抗菌药物的使用和二重感染的发生,值得在临床上广泛使用。
【Abstract】 Objective: To evaluate the clinical significance of C-reactive protein and procalcitonin in the diagnosis and treatment of chronic obstructive pulmonary disease( COPD). Methods: 164 patients with COPD were selected in respiratory department of our hospital during March,2011 to June,2013,included acute exacerbation period( AECOPD) 84 cases and stable period 80 cases. Through detecting the expression of procalcitonin and C-reactive protein,the clinical significance of COPD diagnosing was evaluated. Meanwhile,following the treatment guidance based on PCT and CRP levels,the guiding role of PCT and CRP in COPD treatment was accessed. Results:Compared with the COPD group,the expression levels of PCT and CRP in AECOPD group were significantly higher. The efficiency of the treatment of COPD has no statistically significant difference between guide therapy group and conventional therapy group,the incidence of AECOPD in patients treated with guidance was decreased than that in conventional therapy group,but there was no significant difference between the two groups; but for the duration of hospitalization,antibiotic treatment time,superinfection rates,those in guide treatment group were significantly reduced compared with conventional therapy group. Conclusion: PCT,CRP can be used as an early serological marker to diagnose AECOPD,and the dynamic detection of PCT,CRP level can be used to guide clinical treatment of COPD,it can shorten the duration of hospitalization,and reduce the use of antibiotics and the occurrence of dual infection.
