【摘要】 目的探讨双套管切割活检针联合康派特医用胶在肺中央型病变患者CT引导下经皮肺穿刺活检术中的可行性及安全性。方法选择2011年1月—2014年1月河北医科大学第一医院胸外科收治的肺中央型病变患者100例,随机分为试验组和对照组,各50例。试验组患者采用双套管切割活检针进行CT引导下经皮肺穿刺活检,并采用康派特医用胶封闭穿刺针道;对照组患者采用18G-Gallini切割活检针进行CT引导下经皮肺穿刺活检。比较两组患者穿刺成功率、诊断准确率、气胸及出血发生情况。结果两组患者穿刺成功率均为100%;试验组患者诊断准确率为94%,对照组为92%,差异无统计学意义(P>0.05)。试验组患者气胸量<10%发生率、气胸发生率、单纯针道出血发生率及出血发生率均低于对照组(P<0.05);两组患者气胸量≥10%发生率、渗血并咯血发生率、痰中带血发生率及少量血胸发生率比较,差异无统计学意义(P>0.05);两组患者均未出现过敏等不良反应。结论双套管切割活检针联合康派特医用胶应用于肺中央型病变患者CT引导下经皮肺穿刺活检术安全、可行,诊断准确率较高。更多还原

【Abstract】 Objective To investigate the feasibility and safety of double pipe cutting biopsy needle combined with COMPACTO medical adhesive in CT guided percutaneous lung biopsy in patients with central pulmonary lesions. Methods From January 2011 to January 2014,a total of 100 patients with central pulmonary lesions were selected in the Department of Thoracic Surgery,the First Hospital of Hebei Medical University,and they were randomly divided into control group and test group,each of 50 cases. Patients of control group received CT guided percutaneous lung biopsy by using 18G-Gallini cutting biopsy needle,while patients of test group received CT guided percutaneous lung biopsy by using double pipe cutting biopsy needle combined with COMPACTO medical adhesive. Success rate of puncture, accuracy rate of diagnosis, incidence of aerothorax and hemorrhage were compared between the two groups. Results The success rate of puncture of the two groups were both 100%; the accuracy rate of diagnosis of control group was 92%, that of test group was 94%, the difference was not statistically significantly different( P > 0. 05). The incidence of < 10% of aerothorax volume,aerothorax,needle bleeding and total hemorrhage events of test group were statistically significantly lower than those of control group( P < 0. 05); while no statistically significant differences of ≥10% aerothorax volume,errhysis complicated with hemoptysis,blood- stained sputum or small- amount hemothorax was found between the two groups( P > 0. 05). No one of the two groups occurred allergy or other adverse reactions. Conclusion Double pipe cutting biopsy needle combined with COMPACTO medical adhesive is feasible and safe in CT guided percutaneous lung biopsy in patients with central pulmonary lesions,has high accuracy rate of diagnosis.更多还原
