机器人立体定向辅助系统治疗中等量丘脑出血的临床疗效研究
Clinical Effect of Stereotactic Robot- assisted System on Moderate Amount Thalamic Hemorrhage
【摘要】 目的探讨机器人立体定向辅助系统治疗中等量丘脑出血(TH)的临床疗效。方法选取2010年1月—2014年12月延安大学附属医院东收治的98例中等量TH患者,随机分为对照组和观察组各49例。对照组采用单纯内科保守治疗,观察组采用机器人立体定向辅助系统进行手术治疗,比较两组患者治疗后的临床效果。结果两组患者性别、年龄、血肿量、发病-手术时间、破入脑室例数、入院时的格拉斯哥预后量表(GOS)评分和美国国立卫生研究院卒中量表(NIHSS)评分比较,差异无统计学意义(P>0.05)。治疗后30 d,观察组患者GOS评分和转归良好率优于对照组;治疗后1、2、3个月,观察组患者NIHSS评分均低于对照组(P<0.05);治疗后3个月,观察组患者改良Rankin量表(mRS)评分优于对照组(P<0.05)。结论与内科保守治疗相比,采用机器人立体定向辅助系统治疗中等量TH,可以加快患者血肿的清除速度,且患者近期神经功能恢复和转归情况较好。
【Abstract】 Objective To explore clinical effect of stereotactic robot- assisted system on moderate amount thalamic hemorrhage. Methods A total of 98 patients with thalamic hemorrhage were selected in Affiliated Hospital of Yan’an University of Dongguan Cardiovascular Ward from January 2011 to December 2012,and they were randomly divided into control group( n =49) and experiment group( n = 49). Patients of control group were treated with department of internal medicine conservative treatment,while patients of observation group were treated with stereotactic robot- assisted system. The clinical effect was compared between the two groups. Results No statistically significant differences of gender, age, hematoma volume,incidence and duration of surgery,and the number of cases of rupture into ventricles,GOS score and NIHSS score were found among the two groups before treatment( P > 0. 05). The GOS score,turnover rate was better than that of control group after treatment of 30 d( P < 0. 05); the NIHSS score was lower than that of control group after treatment of 1,2,3 month( P <0. 05); the mRS scroe was better than that of control group( P < 0. 05) after treatment of 3 months. Conclusion Compared with conservative internal medicine,moderate amount thalamic hemorrhage treated with stereotactic robot- assisted system,can speed up the removal rate of patients with hematoma, and has good effect on neurological function recovery and prognosis of patients.
