医院新生儿耐甲氧西林金黄色葡萄球菌感染危险因素分析
Risk factors analysis of methicillin-resistant Staphylococcus aureus in hospital newborns
【摘要】 目的分析医院新生儿耐甲氧西林金黄色葡萄球菌(MRSA)感染发生的危险因素,为采取有效措施预防和控制MRSA医院感染提供科学依据。方法选取189例感染MRSA的新生患儿作为实验组,72例未感染MRSA的新生患儿作为对照组,收集病历信息及相关临床资料,通过运用非条件Logistic回归和卡方检验进行统计,分析医院新生儿MRSA感染的危险因素。结果经单因素统计分析结果显示:实验组与对照组比较,侵入操作以及血清蛋白降低之间的差异无统计学意义(P>0.05),而两组之间应用抗生素种类、实验室指标(合并其他病毒感染、白细胞降低)、住院时间>3周、是否入住ICU以及是否使用激素之间的差异具有统计学意义(P<0.05)。多因素分析结果显示,应用抗生素种类数、合并感染其他病毒、住院时间>3周以及使用激素与医院出生新生儿感染MRSA具有相关性。结论应用抗生素种类数、合并感染其他病毒、住院时间>3周、入住ICU以及使用激素是医院新生儿感染MRSA的危险因素。了解医院新生儿MRSA感染的危险因素,可以从多途径控制危险因素,从而降低耐药率发生。
【Abstract】 Objective To analyze the risk factors for hospital neonatal methicillin- resistant Staphylococcus aureus( MRSA) infection,in order to provide a scientific basis for taking effective measures to prevent and control MRSA hospital infections. Methods Medical information and clinical data of 189 MRSA infected newborns( experiment group) and 72 healthy newborns( control group) were collected. The risk factors for hospital neonatal MRSA infection were analyzed. Non conditional logistic regression and chi- square test were used for statistic analysis. Results Univariate statistical analysis showed that the differences in invasive operations and reduced serum protein were not statistically significant between two groups( P > 0. 05); however,the statistical differences were found in the type of antibiotics used,virus infection,leukopenia,length of hospital stay > 3 weeks,ICU stay and the use of hormones between two groups( P < 0. 05). Multivariate analysis showed that the number of types of antibiotics used,viral infection,length of hospital stay > 3 weeks and the use of hormones were relevant to hospital neonatal MRSA infection. Conclusion Several types of antibiotics,viral infection,length of hospital stay > 3 weeks,ICU stay and the use of hormones are risk factors for neonatal MRSA infection. Risk factors should be prevented by multi- channel control measures to reduce the occurrence of drug resistance.
