【摘要】 目的探讨老年高血压患者血压变异性(blood pressure variability,BPV)对臂-踝动脉脉搏波传导速度(brachial-ankle pulse wave velocity,ba PWV)的影响。方法入选2013年1月至2014年1月入住老年病科及健康体检老年高血压患者162例,年龄60~82岁,平均(70.5±6.4)岁,患者行动态血压监测、ba PWV及常规生化检查,根据ba PWV,分为高ba PWV(≥1400 cm/s)组88例与低ba PWV组(<1400 cm/s)74例,比较2组间基础资料及动态血压及变异性指标,采用Logistic回归分析探讨BPV与ba PWV的关系。结果高ba PWV组人群年龄、空腹血糖(FBG)、低密度脂蛋白胆固醇(LDL-C)、高敏C反应蛋白(CRP)、同型半胱氨酸(HCY)均高于低ba PWV组,高ba PWV组动态血压24 h平均脉压、日间平均脉压、夜间平均脉压、夜间收缩压标准差、日间舒张压标准差、舒张压变异性、舒张压变异系数高于低ba PWV组。校正年龄、FBG、LDL-C、CRP、HCY后,回归分析显示日间平均脉压、夜间平均脉压、24 h平均脉压、舒张压变异性、舒张压变异系数为ba PWV的独立危险因素。结论老年高血压患者BPV与ba PWV相关。更多还原

【Abstract】 Objective To investigate the influence of blood pressure variability( BPV) and brachial-ankle pulse wave velocity( ba PWV) in the elderly hypertensive patients. Methods From January 2013 to January 2014,a total of162 elderly patients with hypertension( > 60 years) who were hospitalized in the department of geriatrics or received health examination were studied. All subjects underwent dynamic blood pressure monitoring, detection of ba PWV, and conventional biochemical examination. According to ba PWV,they were divided into high ba PWV( 1400 cm / s or higher)group and low ba PWV group( lower than 1400 cm / s),and the basic data,dynamic blood pressure and the variability index were compared between the two groups. Logistic regression analysis was used to analyze the relationship between BPV and ba PWV. Results Age,fasting glucose,low-density lipoprotein cholesterol,high-sensitivity C-reactive protein and homocysteine in high ba PWV group were higher than those in low ba PWV group. 24 h ambulatory blood pressure pulse pressure,average pulse pressure in the day,at night,on average,nighttime systolic blood pressure standard deviation,daytime diastolic blood pressure variability standard deviation,diastolic blood pressure,diastolic blood pressure variation coefficient were higher in high ba PWV group than those in low ba PWV group. Regression analysis showed that,after correcting age,fasting glucose,low density lipoprotein,high-sensitivity c-reactive protein and homocysteine,daytime or night or 24 hours average pulse pressure,diastolic blood pressure,diastolic blood pressure variation coefficient were the independent risk factors for ba PWV. Conclusions The blood pressure variability in the elderly patients with hypertension is associated with brachial-ankle pulse wave velocity.更多还原
