安氏Ⅱ类下颌后缩儿童应用Twin-block矫治器功能矫治前后上气道变化的锥形束CT研究
A CBCT study on the upper airway of the children with Class Ⅱ mandibular retrusion before and after functional treatment by Twin-block appliance
【摘要】 目的:探讨安氏Ⅱ类下颌后缩儿童患者功能矫治前后上气道的三维变化。方法:用Twin-block矫治器治疗下颌后缩儿童患者,拍摄其矫治前后的锥形束CT,应用Dolphin 11.5软件进行三维测量分析,采用配对t检验对安氏Ⅱ类下颌后缩患者功能矫治前后上气道的变化进行比较。结果:患者上气道的总体积、腭咽、舌咽、喉咽、口咽体积,软腭尖平面(SP)、会厌顶平面(TE)上气道的截面积,口咽段最小截面积,SP矢状径、横径,TE横径均显著增大(P<0.05);气道形态在SP趋于圆形,而在TE趋于扁平(P<0.05)。结论:Twin-block功能矫治能使下颌后缩儿童上气道增大,改善呼吸功能。
【Abstract】 Objective: To study the changes of upper airway dimension and morphology in Class Ⅱ mandibular retrusion children after functional treatment by Twin-block appliance. Methods: The Cone-Beam CT( CBCT) data of upper airway of the subjects were measured with Dolphin 11. 5 software before and after fuctional treatment by Twin-block appliance. The comparison of upper airway dimension and morphology between pre-treatment and post-treatment was performed by paired t-text. Results: After functional treatment the volume of total upper airway,velopharyngeal airway,glossopharyngeal airway,laryngopharyngeal airway,oropharyngeal airway,the sectional area of the inferior of the soft palate( SP),tip of the epiglotti( TE),the sagittal diameter of SP,the lateral diameter of SP and TE increased( P < 0. 05) in the children; the airway’s shape of SP was attended to be rounder and TE to be more flat. Conclusion:Functional treatment is effective in the treatment of skeletal Class Ⅱ mandibular retrusion of children by increase of the upper airway and improvement of respiration.
